摘 要:目的探讨米力农雾化治疗重症肺炎并心力衰竭患儿的效果及其对血清脑利钠肽(BNP)水平、心功能的影响。方法选取2016年6月至2018年6月该院收治的116例重症肺炎并心力衰竭患儿作为研究对象,采用随机数表法分为观察组和对照组,每组各58例。两组患儿均予以常规治疗,观察组在此基础上加用米力农雾化。比较两组患儿临床疗效和症状体征改善时间,评估治疗前及治疗7 d后患儿心功能[左心室舒张末期内径(LVDd)、左心室收缩末期内径(LVSd)、左心室射血分数(LVEF)]、实验室指标[BNP、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)],记录两组患儿药物不良反应发生率。结果治疗7 d后,观察组患儿总有效率高于对照组(94.83%vs.74.14%,χ2=9.469,P<0.01)。观察组患儿心率正常、呼吸正常、湿啰音消失、肝脏回缩、烦躁消除时间均明显短于对照组(P<0.05)。治疗7 d后,两组患儿LVDd、LVSd、BNP、TNF-α均明显低于治疗前(P<0.05),LVEF均明显高于治疗前(P<0.05),且观察组变化幅度明显大于对照组(P<0.05)。结论米力农雾化治疗重症肺炎并心力衰竭安全有效,有利于促进患儿康复、改善其心功能及相关实验室指标水平。Objective To explore the efficacy of milrinone atomization therapy for treating children patients with severe pneumonia complicating heart failure and its effects on serum brain natriuretic peptide(BNP)level and cardiac function.Methods Total 116 children patients with severe pneumonia complicating heart failure admitted to the hospital from June 2016 to June 2018 were selected as the study subjects and divided into the observation group and control group according to the random number table method,58 cases in each group.The two groups were given the routine treatment,and on this basis the observation group was added with the milrinone atomization.The clinical efficacy and improvement times of symptoms and signs were compared between the two groups.And the cardiac function[left ventricular end diastolic diameter(LVDd),left ventricular end systolic diameter(LVSd),left ventricular ejection fraction(LVEF)]and laboratory indicators[BNP,tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]were evaluated before treatment and after 7 d treatment.The incidence rates of adverse drug reactions were recorded in the two groups.Results After 7 d treatment,the total treatment effective rate in observation group was higher than that in the control group(94.83%vs.74.14%,χ2=9.469,P<0.01).The time of normal heart rate,normal breathing,wet rales disappearance,liver retraction and dysphoria elimination in the observation group was significantly shorter than that in the control group(P<0.05).After 7 d treatment,LVDd,LVSd,BNP and TNF-αin the two groups were significantly lower than those before treatment(P<0.05),while LVEF was significantly higher than that before treatment(P<0.05),moreover the change amplitude in observation group was significantly better than that in the control group(P<0.05).Conclusion The milrinone atomization therapy is safe and effective in the treatment of severe pneumonia complicating heart failure,and is conducive to the rehabilitation of children patients and the improvement of cardiac function and related
