摘  要:术后恶心呕吐(PONV)是最常见的手术和麻醉后并发症,临床上常采用Apfel预测量表来评估患者发生PONV的风险。目前的临床研究和基础实验证明,许多基因的单核苷酸变性,如htr3b rs3758987、Kcnb2 rs349358、Chrm3 rs2165870等都是PONV的独立预测因子,提示PONV的发生具有遗传倾向。此外,5-HT3B基因的缺失变性,Oct1的基因突变以及CYP2D6代谢系统基因变性等,可以解释止吐药疗效的个体差异性,提示基因多态性对于PONV的临床预防和治疗有着指导意义。Postoperative nausea and vomiting(PONV)is the most common complication after operation and anesthesia.Apfel predictive scale is commonly used to evaluate the risk of PONV in patients.Current clinical studies and basic experiments have proved that the single nucleotide denaturation of many genes,such as htr3b rs3758987,Kcnb2 rs349358,Chrm3 rs2165870 and so on,are independent predictors of PONV,suggesting that PONV has a genetic tendency.In addition,the deletion and degeneration of 5-HT3B gene,the gene mutation of Oct1 gene and the gene degeneration of CYP2D6 metabolic system can explain individual differences in the efficacy of antiemetic drugs,suggesting that genetic polymorphism has guiding significance for the clinical prevention and treatment of PONV.
