八脉交会穴配穴温针疗法治疗颈型颈椎病的临床研究

摘　　要：目的观察八脉交会穴配穴温针疗法治疗颈型颈椎病疗效。方法将80例患者按随机数字表法分为观察组和对照组各40例。对照组常规针刺。患者俯卧位,取病变节段颈椎夹脊穴、风池、大椎、肩井、肩外俞、列缺,穴位定位参照《针灸学》,常规消毒穴位处皮肤,采用0.30 mm×25 mm一次性无菌针灸针,其中肩井向后方45°斜刺;夹脊穴向脊中斜刺;肩井平刺;其他穴位直刺;得气后行提插捻转,平补平泻法,10 min行针1次,留针25 min后,出针后予以红外线照射颈部,20 min/次,隔天1次。观察组八脉交会穴配穴温针疗法。患者取舒适俯卧位或坐位,在对照组基础上加取后溪(双侧)、申脉得气后,接通YY-1型介入温针仪,调节输出强度以患者舒适为度,留针20~30 min/次,隔天1次。连续治疗10次为1疗程。观测临床表现、健康状况调查简表(SF-36评分)、Northwick Park颈痛量表(NPQ评分)、视觉模拟评分法(VAS评分)、不良反应。连续治疗2疗程(20次),判定疗效。结果观察组临床痊愈22例,显效10例,有效6例,无效2例,总有效率(95.00%,38/40);对照组临床痊愈17例,显效9例,有效6例,无效8例,总有效率80.00%(32/40);观察组疗效优于对照组(P<0.05)。体征积分、SF-36评分、NPQ评分、VAS评分两组均有改善(P<0.05),观察组改善优于对照组(P<0.05)。SF-36评分各维度两组均有改善(P<0.05),观察组改善优于对照组(P<0.05)。结论八脉交会穴配穴温针疗法治疗颈型颈椎病,疗效满意,可降低疼痛程度,改善生活质量,值得推广。Objective To observe the curative effect of Bamai Jiaohui points combined with warm needling therapy on cervical spondylosis.Method Eighty patients were divided into observation group and control group according to random number table method,40 cases in each group.The control group received routine acupuncture.In the prone position,the patients took Jiaji points,Fengchi(GB20),Dazhui(DU14),Jianwaishu(SI14)and Lieqian(LU7)at the diseased segment of cervical vertebra as reference to Acupuncture and Moxibustion,routinely disinfected the skin at the points,and we used 0.30 mm×25 mm disposable sterile acupuncture needles,45°oblique needling at the back of shoulder well,oblique needling at Jiaji points to the middle of the spine,horizontal needling at shoulder well.We used the lifting,inserting and twisting as well as reinforcing and reducing manipulation,needling every 10 minutes and retaining 25 minutes.We used infrared irradiation of neck after needling,20 minutes/time,once every other day.The observation group was treated with warming needling at Bamai Jiaohui points.The patients took comfortable prone position or sitting position.On the basis of the control group,Houxi(bilateral,SI3)and Shenmai(BL62)were added.After getting Qi,the YY-1 interventional thermo-needle instrument was connected to adjust the output intensity to the patients'comfort.The needle was retained 20-30 minutes per time,once every other day.Continuous treatment for 10 times was a course of treatment.The clinical manifestations,health status questionnaire(SF-36 score),Northwick Park Neck Pain Scale(NPQ score),visual analogue score(VAS score)and adverse reactions were observed.After continuous treatment for 2 courses(20 times),the efficacy was determined.Result A total of 22 cases in the observation group were cured,10 cases were markedly effective,6 cases were effective,2 cases were ineffective,and the total effective rate was 95.00%.In the control group,17 cases were cured,9 cases were markedly effective,6 cases were effective,8 cases were inef
