摘　　要：目的提高对盆腔脂肪增多症的诊治水平。方法回顾性分析4例盆腔脂肪增多症的临床症状、影像学表现和治疗结果,结合文献复习讨论盆腔脂肪增多症诊治特点。结果2例患者行盆腔脂肪清除输尿管膀胱再植术。术中见盆腔脂肪组织明显增多,膀胱输尿管周围充满大量脂肪组织。术后复查IVU示肾盂输尿管积水减轻,血肌酐下降。1例患者行膀胱电切手术治疗腺性膀胱炎及膀胱出口梗阻,继续随访观察病情变化。1例行回肠通道手术,术后出现吻合口狭窄,应用球囊扩张治疗效果显著。结论CT及MRI为本病的主要诊断依据,目前关于盆腔脂肪增多症的治疗尚无统一路径,应根据患者情况设计个体化治疗方案。定期随访十分重要。Objective To improve the diagnosis and treatment of pelvic fat.Methods The clinical symptoms,imaging manifestations and treatment results of 4 cases of pelvic steatomas were retrospectively analyzed,and the characteristics of diagnosis and treatment of pelvic steatomas were discussed in conjunction with the literature review.Results Two patients underwent pelvic fat removal and ureteral bladder replantation.During the operation,a significant increase in pelvic adipose tissue was observed,and a large amount of adipose tissue was filled around the bladder and ureter.Postoperative review of IVU showed that the hydronephrosis of the renal pelvis and ureter decreased,and serum creatinine decreased.One patient underwent resection of the bladder to treat glandular cystitis and bladder outlet obstruction,and continued to observe the changes in the condition.One patient underwent ileal passage surgery,and anastomotic stenosis appeared after surgery.The effect of balloon dilatation was significant.Conclusion CT and MRI are the main diagnostic basis of the disease.At present,there is no unified path for the treatment of pelvic fatosis,and an individualized treatment plan should be designed according to the situation of the patient.Regular follow-up is important.
