2012年-2019年临床输血病例质量调查与持续改进调查分析

摘　　要：目的通过对临床输血病例质量进行定期评估分析,提高输血病例质量,保障临床合理用血水平。方法对本院2012年-2019年输血出院病例进行定期抽查,并对结果进行回顾性分析及比较。结果1496份输血病例中有不合格病例396份,不合格率为26.5%。2012年-2019年不合格病例数呈逐年下降的趋势,差异有统计学意义(P<0.05)。输血前评估、用血申请单、输血治疗知情同意书、输血前5项检测、配血和输血实施、输血后效果评估、医生输血记录完整性等缺陷率呈逐年下降的趋势,差异有统计学意义(P<0.05)。结论通过每月对临床输血病例进行定期抽查,分析和评估临床用血情况,不断持续改进可以提高合理用血水平及输血病例质量。Objective To improve blood transfusion quality and ensure the rational use of blood by evaluating and analyzing the clinical blood transfusion.Methods A retrospective comparison and statistical analysis of transfusion medical records of discharged patients of our hospital from 2012 to 2019 were carried out regularly.Results There were 396 unqualified blood transfusion in 947 blood transfusion cases,and the unreasonable rate was 26.5%.From 2012 to 2019,the unqualified rate decreased year by year(P<0.05).The pre-transfusion evaluation,blood requisition,blood transfusion informed consent,pre-transfusion 5 tests,matching and transfusion implementation,integrity of doctor’s blood transfusion records and so on were decreasing year by year,and the differences were statistically significant(P<0.05).Conclusion Through regular sampling of clinical blood transfusion cases monthly,analysis and evaluation of clinical blood use,and continuous improvement can improve the level of rational blood use and the quality of transfusion.
