摘 要:目的研究血糖浓度、胱抑素C(Cys-C)联合24 h尿蛋白(24 h UmALB)检测在诊断糖尿病合并肾功能衰竭中的应用价值,以期为临床早期防治提供参考。方法选择2016年10月-2018年10月于本院接受诊治的184例糖尿病患者作为观察对象,其中单纯糖尿病组95例、糖尿病合并肾功能衰竭组89例;另选择同期于本院接受健康体检的80例正常人员作为健康对照组;对3组人员的血糖水平、Cys-C、24 h UmALB表达情况进行检测,探讨各项指标单独检测、联合检测糖尿病合并肾功能衰竭的阳性率。结果单纯糖尿病组、糖尿病合并肾功能衰竭组患者的血清HbA1c、FBG、Cys-C及24 h UmALB水平均高于健康对照组,糖尿病合并肾功能衰竭组高于单纯糖尿病组,差异有统计学意义(P<0.05);随着糖尿病患者肾功能衰竭严重程度的增加,血清HbA1c、FBG、Cys-C及24 h UmALB水平呈升高趋势(P<0.05);联合检测糖尿病合并肾功能衰竭的阳性率高于HbA1c、FBG、Cys-C、24 h UmALB单独检测,差异有统计学意义(P<0.05)。结论血糖浓度、胱抑素C联合24 h尿蛋白检测在诊断糖尿病合并肾功能衰竭中的应用价值高,联合检测上述治疗表达情况,能为临床诊断糖尿病合并肾功能衰竭及判定病情进展提供信息,值得临床推广。Objective To analyze the application value of blood glucose concentration,cystatin C(Cys-C)combined with 24 h urine protein(24 h UmALB)in the diagnosis of diabetic renal failure,in order to provide reference for early clinical prevention.Methods A total of 184 patients with diabetes mellitus who were admitted to our hospital from October 2016 to October 2018 were enrolled.Among them,95 cases of patients in the diabetes group(group A)and 89 patients with diabetes mellitus complicated with renal failure(group B).Another 80 cases of normal people who received a physical examination at the same time as the control group were selected.The blood glucose levels,Cys-C and 24 h UmALB expressions of the three groups were tested.The positive rate of each index was detected and combined with the detection of diabetic renal failure.Results The serum levels of HbA1c,FBG,Cys-C and 24 h UmALB in group A and group B were higher than those in the control group(P<0.05).The serum levels of HbA1c,FBG,Cys-C and 24 h UmALB increased with the severity of renal failure in diabetic patients(P<0.05).The positive rate of combined detection of diabetic renal failure was higher than that of HbA1c,FBG,Cys-C,and 24 h UmALB,with the differences statistically significant(P<0.05).Conclusion The blood glucose concentration,cystatin-C combined with 24 h urine protein detection has high value in the diagnosis of diabetic renal failure.Combined detection of the above treatment expression can provide information for clinical diagnosis of diabetes with renal failure and to determine the progress of the disease,which is worthy of clinical promotion.
