手足口病并发脑炎患儿血清白细胞介素-10、超敏C-反应蛋白及肿瘤坏死因子-α指标变化及临床意义

摘　　要：目的研究手足口病并发脑炎患儿血清白细胞介素-10(IL-10)、超敏C-反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)指标变化。方法选择2017年3月-2018年11月本院165例手足口病患儿,根据是否并发脑炎分成未并发脑炎组74例、并发脑炎组91例,并发脑炎组中47例为普通型、44例为重症型;选择80例正常儿童作为对照组。分析普通型、重症型手足口病并发脑炎患儿血清IL-10、hs-CRP、TNF-α水平变化与相关性。结果未并发脑炎组、并发脑炎组患儿的血清IL-10、hs-CRP、TNF-α水平高于对照组,并发脑炎组高于未并发脑炎组(P<0.05);重症型手足口病并发脑炎患儿血清IL-10、hs-CRP、TNF-α高于普通型患儿(P<0.05);手足口病并发脑炎患儿的血清IL-10和hs-CRP表达、TNF-α表达均呈正相关(r值分别为0.439、0.398,P<0.05);且hs-CRP与TNF-α表达呈正相关(r=0.417,P<0.05)。结论血清IL-10、hs-CRP、TNF-α检测利于判定手足口病并发脑炎严重程度、评估预后。Objective To study the changes of serum interleukin-10,high-sensitivity C-reactive protein and tumor necrosis factor-αin children with hand-foot-mouth disease complicated with encephalitis.Methods Between March 2017 and November 2018,165 cases of children with hand-foot-mouth disease were selected as subjects.All the children were divided into 74 cases without encephalitis group and 91 cases with encephalitis group according to whether they were complicated by encephalitis.In the encephalitis group,47 cases were common type and 44 cases were severe type;80 cases of normal children were selected as the control group.To analyze the changes and correlations of serum IL-10,hs-CRP and TNF-αlevels in children with severe hand-foot-mouth disease complicated with encephalitis.Results The levels of serum IL-10,hs-CRP and TNF-αin the unencumbered encephalitis group and the encephalitis group were higher than those in the control group,and the encephalitis group was higher than the non-concurrent encephalitis group(P<0.05);serum IL-10,hs-CRP and TNF-αwere significantly higher in children with severe hand-foot-mouth disease complicated with encephalitis than in normal children(P<0.05).The expressions of serum IL-10,hs-CRP and TNF-αin the encephalitis group were positively correlated(r=0.439,r=0.398,P<0.05),and the expressions of hs-CRP and TNF-αwere positively correlated(r=0.417,P<0.05).Conclusion The detection of serum IL-10、hs-CRP and TNF-is helpful to determine the severity of hand-foot-mouth disease complicated with encephalitis and to evaluate the prognosis.
