四磨汤联合麻仁软胶囊治疗混合痔术后便秘临床观察

摘　　要：目的探讨四磨汤口服液联合麻仁软胶囊治疗混合痔术后便秘效果分析。方法将2016年8月-2019年8月在医院肛肠科行混合痔手术的84例术后便秘患者随机分为两组,均采用外剥内扎术治疗,术后均常规给予饮食、排便等指导,在此基础上,对照组麻仁软胶囊治疗,观察组使用四磨汤口服液联合麻仁软胶囊治疗,连续用药10 d后对比两组的临床疗效、术后便秘指标变化、中医证候积分变化、创面恢复效果评分、并发症发生率。结果观察组治疗有效率明显高于对照组(P<0.05);观察组排便次数明显多于对照组,排便时间明显短于对照组,VAS疼痛评分及排便困难评分明显低于对照组(P<0.05);观察组治疗后肛门坠胀疼痛、肛门异物感、便血、便秘等中医证候积分明显低于对照组(P<0.05);观察组治疗后创面渗液、疼痛、水肿、创面面积、创面肉芽组织等评分明显低于对照组(P<0.05);观察组肛门出血、肛门渗液、肛门失禁、痔疮复发等并发症发生率明显低于对照组(P<0.05)。结论四磨汤口服液联合麻仁软胶囊治疗混合痔术后便秘效果显著,能有效缓解便秘症状,促进排便,减轻排便困难及排便疼痛,促进术后肛门水肿、疼痛的快速消退,加速康复进程,提升术后恢复效果,具有积极的临床意义。Objective To explore the effect of Simo decoction oral liquid combined with Maren soft capsule in the treatment of constipation after operation of mixed hemorrhoids.Methods 84 cases of postoperative constipation were randomly divided into two groups from August 2016 to August 2019.All of them were treated with external stripping and internal ligature,and all of them were given routine guidance such as diet,defecation and so on.The control group was treated with Ma Ren soft capsule,while the observation group was treated with Simo decoction oral liquid combined with Ma Ren soft capsule.Compared the two groups with the clinical efficacy,postoperative constipation index,TCM syndrome score and wound recovery effect.Result The effective rate of the observation group,which was higher than that of the control group (P<0.05).The number of defecation in the observation group was significantly higher than that of the control group,and the defecation time was significantly shorter than that of the control group (P<0.05).After the treatment of the observation group,the symptoms of the syndrome of the traditional Chinese medicine,such as the pain of the anus,the body sensation of the anus,the hematochezia and the constipation,were significantly lower than that of the control group (P<0.05).The scores of wound exudate,pain,edema,wound area and granulation tissue of the observation group were significantly lower than that in the control group (P<0.05).The incidence of anal hemorrhage,anal effusion,anal incontinence,and recurrence of hemorrhoids in the observation group was significantly lower than that in the control group (P<0.05).Conclusion Simo decoction oral liquid combined with Maren soft capsule is effective in the treatment of constipation after mixed hemorrhoids,which can effectively relieve constipation symptoms,promote defecation,relieve defecation difficulty and defecation pain,promote postoperative anal edema,rapid regression of pain,accelerate rehabilitation process and improve postoperative recovery effect.
