摘 要:目的研究慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者应用无创呼吸机治疗的临床效果。方法 144例慢性阻塞性肺疾病伴有呼吸衰竭患者,随机分为对照组和治疗组,每组72例。对照组患者采用常规治疗,治疗组患者在对照组治疗基础上采用无创呼吸机治疗。比较两组血气指标改善效果、治疗3 d后插管情况及插管患者性别、年龄、病程。结果治疗组血气指标改善总有效率91.67%高于对照组的62.50%,差异具有统计学意义(P<0.05)。治疗组患者治疗3 d后插管率8.33%低于对照组的23.61%,差异具有统计学意义(P<0.05)。治疗组插管患者平均年龄、平均病程均高于对照组,差异具有统计学意义(P<0.05);两组插管患者性别比较差异无统计学意义(P>0.05)。结论慢性阻塞性肺疾病合并呼吸衰竭患者应用无创呼吸机治疗可有效改善患者的病情,改善临床症状及血气指标,提高治疗效果,治疗后插管率低,值得推广。Objective To study the clinical effect of non-invasive ventilator therapy for chronic obstructive pulmonary disease(COPD) complicated by respiratory failure. Methods A total of 144 patients with chronic obstructive pulmonary disease and respiratory failure were randomly divided into control group and treatment group, with 72 cases in each group. The control group received conventional therapy, and the treatment group received non-invasive ventilator therapy on the basis of the control group. The improvement effect of blood gas index, intubation status and gender, age, course of treatment of intubation patients after 3 d of treatment were compared between the two groups. Results The total effective rate of blood gas index improvement 91.67% in the treatment group was higher than 62.50% in the control group, and the difference was statistically significant(P<0.05). After 3 d of treatment, the intubation rate 8.33% in the treatment group was lower than 23.61% in the control group, and the difference was statistically significant(P<0.05). The average age and course of treatment of intubation patients in the treatment group were higher than those in the control group, and the difference was statistically significant(P<0.05). There was no statistically significant difference in gender of intubation patients between the two groups(P>0.05). Conclusion Application of non-invasive ventilator can effectively improve the condition of patients with chronic obstructive pulmonary disease and respiratory failure, improve their clinical symptoms, blood gas index, therapeutic effect and survival rate with low intubation rate after treatment, which is worth popularizing.
