前置审方对门诊抗菌药物注射剂使用干预的效果评价

摘　　要：目的考察前置审方对门诊抗菌药物注射剂使用实施干预后,门诊抗菌药物注射剂合理使用情况。方法采用对照、回顾性研究方法,选取前置审方前门诊抗菌药物注射剂处方652张为对照组;运用合理用药软件系统中自定义规则库建立、弹框警示、审方药师干预处方点评与信息药师沟通干预后,选取门诊抗菌药物注射剂处方255张作为试验组。将干预前后的结果进行统计、分析。结果抗菌药物静脉处方占比对照组为0.97%,试验组为0.36%;不合理处方占比对照组为26.99%,试验组为14.12%;处方中抗菌药物金额对照组为373.78元,试验组为330.46元。结论通过前置审方对抗菌药物注射剂使用的干预,可降低门诊抗菌药物静脉输液使用率,减少其不合理处方比率,降低患者药费,促进门诊抗菌药物静脉输液处方质量持续改进。Objective To determine the rational use of antimicrobial injections for outpatients after the intervention of pre-prescription of antimicrobial injections.Methods Totally 652 prescriptions of antimicrobial injections before pre-prescription review were retrospective selected as the control group.After customizing regulation database with prescription automatic screening system,warning,prescription-checking,prescription comment,and communication intervening,255 prescriptions of antimicrobial injections were selected as the experimental group.The results before and after the intervention were analyzed.Results The prescription proportion of intravenous infusion of antibiotics was 0.97%in the control group and 0.36%in the experimental group;the proportion of the unreasonable prescription was 26.99%in the control group and 18.04%in the experimental group;the amount of antibiotics was RMB 373.78 in the control group and RMB 330.46 in the experimental group.Conclusion The pre-prescription intervention on the use of antimicrobial injections helps reduce intravenous infusion of antibiotics,the rates of irrational prescription,the cost of medicine,and improve the quality of antimicrobial intravenous infusion prescription for outpatients.
