中西医结合对老年重症肺炎患者临床疗效及血清学指标的影响

摘　　要：目的:观察血必净注射液联合盐酸氨溴索对老年重症肺炎患者临床疗效及血清学指标的影响。方法:将72例患者按随机数字表法分为对照组(31例)和观察组(41例)。对照组给予西医常规治疗,包括化痰、解痉、吸氧、营养支持、抗炎等,同时以盐酸氨溴索300 mg加入100 m L生理盐水中静脉滴注,每日2次。观察组在对照组治疗的基础上以血必净注射液50 m L加入100 m L生理盐水中静脉滴注,每日2次。两组均治疗14天。观察2组临床疗效及治疗前后血气指标、呼吸力学、炎症因子水平、免疫功能指标的变化情况。结果:总有效率观察组为90.24%(37/41),对照组为64.52%(20/31),2组比较差异有统计学意义(P<0.05)。动脉血二氧化碳分压(partial pressure of carbon dioxide in artery,Pa CO2)、动脉血氧分压(arterial partial pressure of oxygen,Pa O2)、动脉血氧饱和度(saturation arterial blood oxygen,Sa O2)和Pa O2/Fi O2水平等血气指标,C反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子(tumor necrosis factorα,TNF-α)、白细胞介素(interleukin-6,IL-6)等炎症因子水平,CD3^+、CD4^+、CD8^+和CD4^+/CD8^+水平等免疫功能指标,平均气道压(mPaw)、气道阻力(Raw)、吸气峰压(PIP)、气道平台压(Pplat)水平等呼吸力学指标治疗前后2组组内比较,差异有统计学意义(P<0.05);治疗后组间比较差异也有统计学意义(P<0.05)。结论:血必净注射液联合盐酸氨溴索治疗老年重症肺炎,可提高临床疗效,改善血气指标、呼吸力学指标,降低炎症因子水平,提高患者免疫功能,有利于患者康复。Objective:To observe the effects of Xuebijing injection combined with ambroxol hydrochloride on curative effects and serological indicators in the senile patient suffering severe pneumonia.Methods:All 72 patients were allocated to the control group(31 cases)and the observation group(41 cases)according to random number table method.The control group were given conventional therapy of Western medicine,including reducing phlegm,relieving spasm,oxygen inhalation,nutritional support,antiinflammation and others,simultaneously,intravenous dripping of 300 mg,ambroxol hydrochloride and 100 m L,physiological saline,twice each day,and the observation group intravenous dripping of 50 m L,Xuebijing injection and 100 m L,physiological saline,twice each day.Both groups were treated for 14 days.To observe clinical effects,blood gas indexes,respiratory mechanics,the levels of inflammatory factors and immunologic function indexes before and after treating in both groups.Results:Total effective rate of the observation group was 90.24%(37/41),higher than 64.52%(20/31)of the control group,and the difference had statistical meaning(P<0.05).When blood gas indexes including partial pressure of carbon dioxide in artery(PaCO2),arterial partial pressure of oxygen(PaO2),saturation arterial blood oxygen blood(SaO2),PaO2/Fi O2 and others,the levels of inflammatory factors containing C reactive protein(CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6),immunologic function indexes such as CD3^+,CD4^+,CD8^+and CD4^+/CD8^+levels and the indexes of respiratory mechanics including mean airway pressure(m Paw),airway resistance(Raw),peak inspiratory pressure(PIP)and airway platform pressure(Pplat)level were compared before and after treating within two groups,the differences were signficant(P<0.05),the difference had statistical meaning between both groups after treating(P<0.05).Conclusion:Xuebijing injection combined with ambroxol hydrochloride in treating senile patient suffering severe pneumonia could raise clinical effects,improve bloo
