摘　　要：目的了解导致护理人员POCT血糖仪室内质控执行障碍的影响因素。方法采用自设问卷法,自2017年9月—2017年12月对省内89家医院503名护理人员,进行室内质控理论和实践情况调查。结果护理人员室内质控理论、操作流程、目的、频次及失控处理流程合格率分别为35.59%、43.54%、1.39%、19.28%、29.22%;不同层级、职称、职务护士间知识、操作合格率有差异,N1-2高于N0、N3-4,护士和护师高于主管护师以上职称人员,普通护士高于护士长、带教老师、中层管理人员,P<0.05,差异有统计学意义;学历和有无内分泌科室工作经历在理论知识、操作得分差异无统计学意义(P>0.05)。POCT专项培训次数越多、工作年限越短,室内质控目的、质控频次、失控处理措施得分越高,P<0.05,差异有统计学意义。结论导致护理人员室内质控执行障碍的主要原因是质控理论知识及操作流程未掌握、质控目的认知不到位,与POCT专项培训少、高年资护理人员未及时更新知识和医疗机构缺乏完善的管理体系有关,建议对护理人员、尤其是高年资、护士长等管理层护理人员加强POCT血糖仪专项、标准化培训,可以采取多途径、多形式、联合检验科、跨学科培训,提高室内质控执行率。Objective To understand the influencing factors that obstacles to implementation of indoor quality control of POCT blood glucose meter.Methods Using the self-designed questionnaire,503 nurses from 89 hospitals in the province were investigated on the theory and practice of indoor quality control from September 2017 to December 2017.Results The qualified rate of the theory,operation process,purpose,frequency and out of control treatment process of the nursing staff was 35.59%,43.54%,1.39%,19.28% and 29.22%respectively.There were differences in the knowledge and operation qualification rate among nurses of different levels,titles and positions.N1-2 was higher than N0、3-4,nurses and practitioner nurses were higher than those with titles above the supervisor nurses,general nurses were higher than the head nurses,tutor nurses and middle management nurses,P<0.05,the difference was statistically significant;there was no statistically significant difference in theoretical knowledge and operation score between education background and work experience in endocrine department(P>0.05).The higher the number of POCT special training and working years,the higher the score of indoor quality control purpose and frequency and out of control measures,the difference was statistically significant(P<0.05).Conclusion The main reasons that affect the implementation of internal quality control of nursing staff are that the theoretical knowledge and operation process of quality control are not mastered,the importance of quality control is not well understood,which is related to the lack of POCT special training,the lack of timely updating of knowledge of elderly nurses and the lack of a perfect management system in the hospital.It is suggested to strengthen the specialized and standardized training of POCT blood glucose meter for nurses,especially the senior nurses and head nurses,so as to improve the implementation rate of indoor quality control.
