摘 要:目的观察基质金属蛋白酶(MMP)-3抑制剂Ⅰ结合中等强度运动对膝骨性关节炎(KOA)模型大鼠关节软骨的修复效果。方法 48只SD大鼠随机分成正常组、模型组、抑制剂组和抑制剂结合跑台组,每组12只。后3组大鼠膝关节腔内注射碘乙酸钠建立KOA模型,然后抑制剂组大鼠关节腔注射MMP-3抑制剂Ⅰ注射液,抑制剂结合跑台组大鼠在注射抑制剂后再行中等强度跑台训练。采用O’Driscoll组织学评分法分析关节软骨组织及软骨细胞;采用ELISA法测定实验前后大鼠血清MMP-3表达水平;采用免疫组织化学法检测关节软骨Ⅱ型胶原表达水平。结果治疗6周后:(1)抑制剂组和抑制剂结合跑台组大鼠软骨O’Driscoll组织学评分均高于模型组、低于正常组(均P<0.05);抑制剂结合跑台组大鼠软骨O’Driscoll组织学评分高于抑制剂组(P<0.05);(2)抑制剂组和抑制剂结合跑台组大鼠血清MMP-3表达水平均低于模型组,高于正常组(均P<0.05);抑制剂结合跑台组大鼠血清MMP-3表达水平低于抑制剂组(P<0.05);(3)抑制剂组和抑制剂结合跑台组大鼠关节软骨Ⅱ型胶原表达水平均高于模型组(均P<0.05);抑制剂结合跑台组大鼠关节软骨Ⅱ型胶原表达水平高于抑制剂组(P<0.05)。结论 MMP-3抑制剂Ⅰ结合中等强度运动能促进KOA模型大鼠关节软骨的修复。Objective To investigate the effect of MMP-3 inhibitor Ⅰ combined with moderate intensity exercise on the repair of articular cartilage in knee osteoarthritis(KOA) model of rats. Methods Forty eight SD rats were randomly divided into normal group, model group, inhibitor group and inhibitor combined treadmill group with12 rats in each group. KOA model was induced by injecting sodium iodoacetate into the knee joint cavity of SD rats. The normal group and the model group did not receive any treatment. The inhibitor group received MMP-3 inhibitor Ⅰ injection in the articular cavity;while innhibitor combined with treadmill group underwent moderate intensity treadmill training after inhibitor injection. Results After 6 weeks of treatment the cartilage O’Driscoll histological scores of inhibitor group and the inhibitor combined treadmill group were higher than that of model group and lower than that of the normal group(P<0.05). The cartilage O’Driscoll histology score of rats in the inhibitor combined treadmill group was higher than that in the inhibitor group(P<0.05). Serum MMP-3 expression levels in the inhibitor group and the inhibitor combined treadmill group were lower than those in the model group and higher than those in the normal group(P<0.05). The serum expression of MMP-3 of inhibitor-binding treadmill group was lower than that of inhibitor group(P<0.05). The expression levels of type Ⅱ collagen in articular cartilage of rats in the inhibitor group and the inhibitor combined treadmill group were higher than those in the model group(P<0.05). The expression level of typeⅡ collagen in the joint cartilage of rats in the inhibitor combined treadmill group was higher than that in the inhibitor group(P<0.05). Conclusion MMP-3 inhibitorⅠ combined with moderate-intensity exercise can promote the repair of articular cartilage in knee osteoarthritis(KOA) rat model.
