InterTAN与DHS治疗高龄不稳定型股骨转子间骨折临床疗效比较

摘　　要：目的比较股骨近端髓内钉InterTAN与动力髋螺钉(dynamic hip screw,DHS)治疗高龄不稳定型股骨转子间骨折的临床疗效。方法将86例高龄不稳定股骨转子间骨折患者按治疗方式分为Inter TAN组与DHS组,每组43例,分别采用InterTAN与DHS内固定治疗后,观察并比较两组患者手术时间、术中出血量以及术前、术后12个月Harris评分、根据Harris评分得分情况评定的术后12个月时疗效、相关并发症发生率。结果 InterTAN组手术时间、术中出血量、并发症发生率均明显低于DHS组,差异均有统计学意义(均P<0.05)。两组患者术后12个月时的Harris评分均明显高于治疗前,差异均有统计学意义(均P<0.05);InterTAN组术后12个月时的Harris评分明显高于DHS组,差异有统计学意义(P<0.05);InterTAN组疗效优良率高于DHS组,差异有统计学意义(P<0.05)。结论与DHS相比,InterTAN治疗高龄不稳定型股骨转子间骨折操作更为简便,创伤更小,并发症更少,短期疗效优良率、Harris评分更高,可作为优选治疗方法。Objective To compare the clinical effects of proximal femoral intramedullary nail(InterTAN) and dynamic hip screw(DHS) in the treatment of unstable intertrochanteric fracture of femur in the elderly. Methods Eighty six elderly patients with unstable intertrochanteric fracture of femur admitted to our hospital from February 2016 to February 2018 were divided into InterTAN group(n=43) and DHS group(n=43). The operation-related indicators(operation time, intraoperative bleeding volume and Harris score, the short-term excellent and good rate and the incidence of related complications within 12 months after surgery were compared between two groups. Results The operation time, intraoperative bleeding volume and complication rate in the InterTAN group were significantly lower than those in the DHS group(P<0.05);the Harris score 12 months after operation was higher than that before treatment in both groups(P<0.05);while the Harris score of InterTAN group was higher than that of DHS group(P<0.05). The excellent and good rate of InterTAN group was higher than that of DHS group(P<0.05). Conclusion Compared with DHS, InterTAN is more convenient and less traumatic with less complications, higher short-term excellent rate and long-term hip function score in the treatment of unstable intertrochanteric fracture of femur in the elderly.
