摘 要:目的:分析宫颈病变患者挖空细胞形态与人乳头瘤病毒(HPV)感染及其基因型的关系。方法:选取169例宫颈病变患者为研究对象,对其石蜡标本进行24种HPV基因分型检测,以组织病理学诊断作为宫颈病变确诊的金标准,分析其挖空细胞病变、HPV感染情况。结果:169例标本中,挖空细胞阳性率为62.1%(105/169),HPV阳性率为41.4%(70/169);HPV阳性患者挖空细胞阳性率明显高于HPV阴性患者,差异有统计学意义(P<0.05);单一HPV感染占80.0%(56/70),多重HPV感染占20.0%(14/70);高危型HPV感染挖空细胞阳性率与低危型HPV感染挖空细胞阳性率比较,差异无统计学意义(P>0.05)。结论:挖空细胞形态与HPV感染密切相关,挖空细胞形态改变是HPV感染引起宫颈病变的早期事件。Objective: To observe relationship between koilocyte morphology and human papillomavirus(HPV) infection and genotype in patients with cervical lesions. Methods: To detect 24 HPV genotypes in the paraffin-embedded specimens of 169 patients with cervical lesions. Using the histopathological diagnosis as the gold standard for the diagnosis of cervical lesions, the koilocytes lesions and HPV infection were analyzed. Results: Among the 169 specimens, the positive rate of koilocytes was 62.1%(105/169), and the positive rate of HPV was 41.4%(70/169). The positive rate of koilocytes was significantly higher in the HPV-positive patients than in the HPV negative patients;and the difference was statistically significant(P<0.05). Single HPV infection accounted for 80.0%(56/70), multiple HPV infection accounted for 20.0%(14/70). Further, there was no statistically significant difference in the koilocytes positive rate among the high-risk HPV infection patients and the low-risk HPV infection patients. Conclusions: The koilocyte morphology is closely related to HPV infection. The koilocyte morphology change is an early event of cervical lesions caused by HPV infection.
