摘 要:目的探讨泼尼松联合甲氨蝶呤对系统性红斑狼疮患者外周血单个核细胞细胞因子分泌及Th亚群的影响。方法以2017年8月至2019年8月本院收治的70例系统性红斑狼疮患者为研究对象,按电脑数字表法随机分作两组,每组35例。对照组患者予以泼尼松治疗,试验组患者予以泼尼松联合甲氨蝶呤治疗,对比两组外周血单个核细胞细胞因子分泌及Th亚群水平。结果治疗后,试验组患者γ-干扰素为(15.35±2.16)ng/L,白介素10为(10.04±1.21)pg/ml,分别较对照组的(18.51±2.26)ng/L、(14.07±1.02)pg/ml低,差异均有统计学意义(均P<0.05);治疗后,试验组Th17、Tc17、Th1、Tc1均较对照组低,差异均有统计学意义(均P<0.05);治疗后,试验组疾病活动度较对照组低,差异有统计学意义(P<0.05)。结论泼尼松联合甲氨蝶呤可有效对系统性红斑狼疮患者外周血单个核细胞细胞因子分泌及Th亚群进行改善,进而促进患者疾病活动度下降。Objective To investigate the effect of prednisone combined with methotrexate on cytokine secretion and Th subsets in the peripheral blood mononuclear cells of patients with systemic lupus erythematosus.Methods 70 patients with systemic lupus erythematosus admitted to our hospital from August,2017 to August,2019 were enrolled.According to the computer digital table method,they were randomly divided into a control group and an experimental group,with 35 cases in each group.The control group was treated with prednisone,and the experimental group with prednisone and methotrexate.The cytokine secretion and Th subsets of peripheral blood mononuclear cells were compared between the two groups.Results After the treatment,the levels ofγ-interferon and interleukin 10 were lower in the experimental group than in the control group[(15.35±2.16)ng/L vs.(18.51±2.26)ng/L and(10.04±1.21)pg/ml vs.(14.07±1.02)pg/ml;both P<0.05].After the treatment,the levels of Th17,Tc17,Th1,and Tc1 in the experimental group were lower than those in the control group(all P<0.05).After the treatment,the disease activity of the experimental group was lower than that of the control group(P<0.05).Conclusion Prednisone combined with methotrexate can effectively improve the secretion of cytokines and Th subsets in the peripheral blood mononuclear cells of patients with systemic lupus erythematosus,and promote the decline of disease activity.
