带线锚钉结合缝线桥技术治疗TossyⅢ型肩锁关节脱位的可行性及安全性分析

摘　　要：目的探讨带线锚钉结合缝线桥技术治疗TossyⅢ型肩锁关节脱位的可行性及安全性。方法回顾性分析2014年8月至2017年8月收治的TossyⅢ型肩锁关节脱位患者64例的临床资料,按照治疗方法不同分为对照组和观察组,对照组应用锁骨钩钢板内固定治疗,观察组应用带线锚钉结合缝线桥技术治疗,比较2组手术相关指标(手术时间、出血量、切口长度)、手术效果、肩关节活动度及肩关节Constant评分,观察两组并发症发生情况。结果观察组手术时间、住院时间明显短于对照组,术中出血量明显少于对照组,差异具有统计学意义(P<0.01);两组手术效果比较,观察组优良率明显高于对照组,差异有统计学意义(P<0.05);观察组肩关节外展上举活动度及前屈上举活动度明显大于对照组,差异具有统计学意义(P<0.01);观察组疼痛、肌力、功能活动及肩关节活动度评分明显高于对照组,差异具有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异具有统计学意义(P<0.05)。结论带线锚钉结合缝线桥技术治疗TossyⅢ型肩锁关节脱位效果确切,患者出血量少、术后恢复快,有助于缓解患者疼痛,并促进肩关节功能恢复,是一种较为理想的治疗方式,值得临床推广运用。Objective To investigate the feasibility and safety of suture anchor combined with suture bridge technique in treatment of TossyⅢacromioclavicular joint dislocation.Methods The clinical data about 64 patients with TossyⅢacromioclavicular joint dislocation who were treated by surgery in our hospital from August 2014 to August 2017 were retrospectively analyzed.According to different treatment methods,these patients were divided into control group and observation group.The patients in control group were treated by clavicular hook plate internal fixation,however,the patients in observation group were treated by suture anchor combined with suture bridge technique.The surgical related indexes including operation time,hemorrhage volume,length of incision,and surgical effects,shoulder joint activity and shoulder joint Constant score were observed and compared between the two groups.Results The operation time and hospitalization time in observation group were significantly shorter than those in control group,and the intraoperative blood loss in observation group was significantly lower than that in control group(P<0.01).The good/excellent rate in observation group was significantly higher than that in control group(P<0.05).Moreover the shoulder outreach activity and anterior flexion activity in observation group were superior to those in control group(P<0.01).The scores of pain,muscle strength,functional activity and shoulder joint activity in observation group were significantly higher than those in control group(P<0.05).In addition the incidence rate of complications in observation group was significantly lower than that in control group(P<0.05).Conclusion The suture anchor combined with suture bridge technique is effective in treating TossyⅢacromioclavicular joint dislocation,which has the advantages of less bleeding volume and quick recovery after surgery,which is helpful to relieve pain and promote shoulder function recovery.Therefor it is an ideal treatment method and is worthy of clinical promotion.
