摘　　要：范永升教授认为系统性硬化病合并间质性肺病的病位主要在肺脾以及三焦腠理;本病发生的基本病机为脾胃虚弱,少阳三焦衰,肺之腠理间质不得宣通;阳虚寒凝日久则气血不通、经络瘀阻,甚至累及心肾,出现心肾阳虚证候。临床可分为阳虚寒凝、肺失宣降证,气阴不足、肺失宣降证,瘀血阻络、肺失宣降证以及心肾阳虚、肾不纳气证4个基本证型。分别采用温阳散寒、通络宣肺,益气养阴、祛瘀宣肺,活血祛瘀、益气宣肺以及温阳利水、纳气平喘等治法。范教授治疗本病重视培土生金法、温阳散寒法以及宣肺通络法。Professor FAN Yong-sheng believes that positions of systemic sclerosis with interstitial pulmonary disease are lung,spleen,triple energizer and grain of skin and the texture of the subcutaneous flesh.The basic pathogenesis of this disease is the deficiency of spleen and stomach,the weakness of the triple energizer of Shaoyang,and the failure governing diffusion of the grain of skin and the texture of the subcutaneous flesh of lung,the stagnation of qi and blood,stagnation of meridians and collaterals,and even involvement of the heart and kidney,leading to the syndrome of yang deficiency of heart and kidney.The clinical manifestations can be divided into four basic syndromes as yang deficiency and cold coagulation with lung failure governing diffusion,purification and descending,qi and yin deficiency with lung failure governing diffusion,purification and descending,blood stasis and collaterals blockage with lung failure governing diffusion,purification and descending and syndrome of heart and kidney yang deficiency,kidney failure and qi deficiency.They were treated by warming yang and dispelling cold,dredging collaterals and promoting lung,nourishing qi and yin,removing blood stasis and promoting lung,activating blood circulation and removing blood stasis,nourishing qi and promoting lung,warming yang and promoting water,and taking qi and relieving asthma.Professor FAN attached importance to the methods of‘cultivating the soil to produce gold’,warming yang for dispelling cold,promoting lung and dredging collaterals.
