摘  要:目的:探究富血小板血浆(PR P)联合桃红四物汤对激素性股骨头坏死(SA N FH)大鼠股骨头组织中血管内皮生长因子(VEGF)、血小板内皮细胞黏附分子-1(CD31)、碱性磷酸酶(ALP)、β-连环蛋白(β-catenin)蛋白表达的影响。方法:将60只SD大鼠随机分为正常组、模型组、桃红四物汤组、PRP组、联合组。利用内毒素联合激素方法建立SANFH模型,相应给药治疗,治疗6周后处死,并取出双侧股骨头,行HE染色观察组织病理变化,并通过免疫组化法观察VEGF、CD31、ALP和β-catenin蛋白表达情况。结果:与正常组比较,模型组大鼠骨小梁稀疏变细,脂肪细胞体积增大,空骨陷窝大量存在(P<0.01);与模型组比较,各治疗组股骨头中VEGF、CD31、ALP、和β-catenin阳性表达均有不同程度的上调,其中联合组上调水平最为显著(P<0.01,P<0.05)。结论:PRP、桃红四物汤及两者联合治疗可明显上调VEGF、CD31、ALP和β-catenin的蛋白表达,增加股骨头血供,促进坏死区新骨形成,从而有效治疗SANFH。Objective:To investigate the effects of PRP combined with Taohong Siwu Decoction on the expression of VEGF,CD31,ALP andβ-catenin in the femoral head tissues of rats with steroid-induced avascular necrosis of femoral head(SANFH).Methods:A total of 60 SD rats were randomly divided into normal group,model group,Taohong Siwu Decoction group,PRP group and combined group.SANFH model was established by endotoxin combined with hormone.After 6 weeks of treatment,the bilateral femoral head were harvested.The histopathological changes were observed by HE staining,and the expression of VEGF,CD31,ALP andβ-catenin were observed by immunohistochemistry.Results:compared with the normal group,the trabeculae of bone in the model group were thinned and thinned,the volume of fat cells increased,and there were a lot of empty bone lacunae(P<0.01);compared with the model group,the expressions of VEGF,CD31,ALP,andβ-catenin in the femoral head of each treatment group decreased in different degrees,especially in the combined group(P<0.01,P<0.05).Conclusion:PRP,Taohong Siwu Decoction and their combination can significantly up-regulate the expression of VEGF,CD31,ALP andβ-catenin,increase the blood supply of femoral head,promote the formation of new bone in necrosis area,and effectively treat SANFH.
