参松养心胶囊联合西药治疗心力衰竭合并心房颤动临床研究  

Clinical Study on Shensong Yangxin Capsules Combined with Western Medicine for Atrial Fibrillation Complicated with Heart Failure

摘　　要：目的:观察参松养心胶囊联合西药治疗心力衰竭合并心房颤动的临床疗效。方法:选取102例心力衰竭合并心房颤动患者,按随机数字表法分为观察组和对照组各51例。对照组给予西药治疗,观察组在此基础上增加参松养心胶囊治疗,2组均连续治疗8周。对比2组临床疗效、心功能指标、血清学指标及不良反应发生情况。结果:观察组总有效率为88.23%,高于对照组的70.59%,差异有统计学意义(P<0.05)。治疗后,2组左室射血分数(LVEF)均较治疗前升高(P<0.05),左室收缩末径(LVESD)、左室舒张末径(LVEDD)均较治疗前缩小(P<0.05);观察组LVEF高于对照组(P<0.05),LVESD、LVEDD均小于对照组(P<0.05)。治疗后,2组血清B型脑钠肽(BNP)、肌钙蛋白I (cTnI)、肌酸激酶同工酶(CK-MB)水平均较治疗前降低(P<0.05);观察组血清BNP、cTnI、CK-MB水平均低于对照组(P<0.05)。观察组不良反应发生率为19.61%,对照组不良反应发生率为11.76%,组间比较,差异无统计学意义(P>0.05)。结论:参松养心胶囊联合西药治疗心力衰竭合并心房颤动效果显著,能够改善患者的心功能,降低血清BNP、cTnI、CK-MB水平,治疗安全性良好。Objective:To observe the clinical effect of Shensong Yangxin capsules combined with western medicine for heart failure complicated with atrial fibrillation. Methods:A total of 102 cases of patients with heart failure complicated with atrial fibrillation were divided into the observation group and the control group according to the random number table method,51 cases in each group. The control group was treated with western medicine,and the observation group was additionally treated with Shensong Yangxin capsules based on the treatment of the control group. Both groups were treated for eight weeks. The clinical effect,the indexes of heart function and serology,and incidence of adverse reactions in the two groups were compared. Results:The total effective rate was 88.23% in the observation group,higher than that of 70.59% in the control group,the difference being significant(P<0.05). After treatment,the scores of left ventricular ejection fraction(LVEF)in the two groups were higher when compared with those before treatment(P<0.05), and left ventricular end systolic diameter(LVESD) and left ventricular end diastolic diameter(LVEDD) were decreased when compared with those before treatment(P<0.05);the LVEF in the observation group was higher than that in the control group(P<0.05),and both LVESD and LVEDD were less than those in the control group(P<0.05). After treatment,the levels of brain natriuretic peptide(BNP),cardiac troponin I(cTnI) and creatine kinase isoenzyme(CK-MB) in serum in the two groups were decreased when compared with those before treatment(P<0.05);the above three levels in serum in the observation group were lower than those in the control group(P<0.05). The incidence of adverse reactions was 19.61% in the observation group and 11.76% in the control group,there being no significance in the difference between the two groups(P>0.05).
