中药熏蒸联合小针刀治疗强直性脊柱炎临床研究

摘　　要：目的:探讨中药熏蒸联合小针刀治疗强直性脊柱炎的临床疗效。方法:选取强直性脊柱炎患者80例,按随机数字表法分为研究组和对照组各40例。对照组采用小针刀松解粘连法进行治疗,研究组在对照组基础上联合中药熏蒸治疗。观察2组治疗前后的生活质量状况及血清炎性因子水平,统计不良反应发生率和临床总有效率。结果:研究组总有效率为90.0%,高于对照组65.0%(P<0.05)。治疗前,2组生活质量各项评分比较,差异无统计学意义(P>0.05)。治疗后,2组生活质量各项评分较治疗前升高(P<0.05),且研究组生活质量各项评分高于对照组(P<0.05)。治疗前,2组血清炎性因子水平比较,差异无统计学意义(P>0.05)。治疗后,2组血清炎性因子C-反应蛋白(CRP)、白细胞介素-6 (IL-6)、肿瘤坏死因子-α(TNF-α)水平较治疗前降低(P<0.05),研究组血清炎性因子CRP、IL-6、TNF-α水平明显低于对照组(P<0.05)。研究组头晕、感觉障碍、四肢麻木、脊柱敲打反应迟缓等不良反应发生率明显低于对照组(P<0.05)。结论:中药熏蒸联合小针刀松解粘连治疗强直性脊柱炎,可以在一定程度上提高患者的生活质量,降低体内血清炎性因子水平,减少不良反应的发生,值得在临床中推广。Objective: To observe the clinical effect of the therapy of Chinese herbal fumigation combined with small needle-knife for ankylosing spondylitis. Methods: A total of 80 patients with ankylosing spondylitis were divided into the study group and the control group according to the random table number method,40 cases in each group. The control group was treated with small needle-knife method to release the adhesion tendon,and the study group was additionally treated with Chinese herbal fumigation based on the treatment of the control group. The quality of life and the levels of inflammatory cytokines in serum before and after treatment were observed in both groups,and the incidence of adverse actions and the total effective rate were also calculated. Results:The total effective rate was 90.0% in the study group,higher than that of65.0% in the control group(P<0.05). Before treatment,there was no significant difference in the comparison of each score of quality of life between the two groups(P>0.05). After treatment, each score of quality of life in the two groups were increased when compared with those before treatment(P<0.05),and each score in the study group was higher than that in the control group(P<0.05). Before treatment, there was no significant difference in the comparison of the levels of inflammatory cytokines in serum between the two groups(P>0.05). After treatment,the levels of C-reactive protein(CRP),interleukin-6(IL-6), and tumor necrosis factor-α(TNF-α) in the two groups were decreased when compared with those before treatment(P<0.05),and the above scores in the study group were significantly lower than those in the control group(P<0.05). The incidence of adverse reactions such as dizziness,sensory disturbance,limb numbness and slow response to spinal knock in the study group was significantly lower than that in the control group(P<0.05).
