小儿止哮方加减联合布地奈德治疗小儿支气管哮喘发作期临床研究

摘　　要：目的:观察小儿止哮方加减联合布地奈德治疗小儿支气管哮喘的临床疗效。方法:选取96例支气管哮喘发作期患儿,按随机数字表法分为对照组和观察组各48例。对照组在常规治疗基础上给予布地奈德气雾剂治疗,观察组在对照组基础上给予小儿止哮方加减治疗。比较2组临床疗效、临床症状缓解时间、治疗前后白细胞介素-4(IL-4)、白细胞介素-5(IL-5)、肿瘤坏死因子-α(TNF-α)、血管内皮生长因子(VEGF)、免疫球蛋白E(IgE)及人胸腺基质淋巴细胞生成素(TSLP)水平,比较2组治疗前后肺功能指标。结果:治疗前,2组炎症相关指标、肺功能指标比较,差异无统计学意义(P>0.05)。治疗后,2组IL-4、IL-5、TNF-α、IgE、TSLP及VEGF水平均下降,且观察组各指标水平均低于对照组(P<0.05);2组肺功能各指标均较治疗前明显改善,且观察组患儿肺功能改善情况均优于对照组(P<0.05)。治疗后,观察组患儿咳嗽、喘息、呼吸困难、哮鸣音缓解时间均短于对照组,差异有统计学意义(P<0.05)。观察组总有效率为83.33%,高于对照组的62.50%(P<0.05)。结论:小儿支气管哮喘发作期患儿应用小儿止哮方加减联合布地奈德治疗,见效快,疗效好。Objective:To observe the clinical effect of the therapy of modified Xiao'er Zhixiao prescription combined with budesonide for bronchial asthma in children.Methods:A total of 96 cases of children with bronchial asthma were divided into the control group and the observation group according to random number table method,48 cases in each group.The control group was treated with budesonide aerosol based on the treatment of conventional treatment,and the observation group was additionally treated with Xiao'er Zhixiao prescription based on the treatment of the control group.The clinical effect,clinical symptom remission time,and changes in the levels of inflammatory indexes such as interleukin-4(IL-4),interleukin-5(IL-5),tumor necrosis factor-α(TNF-α),vascular endothelial growth factor(VEGF),immunoglobulin E(IgE),and human thymic stromal lymphopoietin(TSLP),and in lung function indexes were compared in both groups.Results:Before treatment,there was no significant difference in the comparison of the inflammatory indexes and lung function indexes between the two groups(P>0.05).After treatment,the levels of IL-4,IL-5,TNF-α,IgE,TSLP and VEGF in both groups were decreased,and the levels of all indexes in the observation group were lower than those in the control group(P<0.05).All lung function indexes in the two groups were significantly improved when compared with those before treatment,and the recovery of lung function in the observation group was better than that in the control group(P<0.05).After treatment,the remission time of cough,pant,dyspnea and wheezing in the observation group was shorter than that in the control group,the difference being significant(P<0.05).The total effective rate was 83.33%in the observation group,higher than that of 62.50%in the control group(P<0.05).Conclusion:The therapy of Xiao'er Zhixiao prescription combined with budesonide has significant therapeutic effect in treating children with bronchial asthma,which has the characteristics of quick effect and great curative effect.
