摘　　要：目的探讨综合护理管理模式对老年急性左心衰竭(acute left ventricular failure,ALVF)合并呼吸机相关肺炎(ventilator associated pneumonia,VAP)的护理效果。方法选取2017年1月—2019年1月在我院确诊住院治疗的老年ALVF合并VAP的患者86例,根据随机数字表法分为对照组和观察组,各43例。对照组采用常规护理,观察组在对照组的基础上采用综合护理管理模式护理,比较2组在护理前后的心肺功能、生命体征变化及患者满意度的差异。结果观察组护理后的氧分压、二氧化碳分压、第1 s用力呼气容积、机械通气时间、发热时间、感染缓解时间、心率、血压、血氧饱和度、呼吸的改善情况均显著优于对照组护理后(P均<0.05);观察组护理满意度为72.09%,显著高于对照组的46.51%,差异有统计学意义(P<0.05)。结论综合护理管理模式的应用能够显著改善老年ALVF合并VAP患者的相关症状,提高护理满意度,效果理想,值得临床推广。Objective To investigate the nursing effect of integrated nursing management model in senile patients of acute left ventricular failure(ALVF)complicated with ventilator associated pneumonia(VAP).Methods Eighty-six senile patients of ALVF combined with VAP who were diagnosed and hospitalized in our hospital from January 2017 to January 2019 were included in the study.According to random number table method,the included patients were divided into control group and observation group,with 43 cases in each group.The control group was given conventional nursing care,and the observation group was given integrated nursing management in addition to the nursing in the control group.Before and after nursing,cardiopulmonary function,vital signs change and the difference of patient’s satisfaction were compared in 2 groups.Results In the observation group,the improvement of partial pressure of oxygen,partial pressure of carbon dioxide in artery,forced expiratory volume in the first second,mechanical ventilation time,fever time,infection remission time,heart rate,blood pressure,blood oxygen saturation and breathing was significantly better than those of the control group after nursing(P<0.05).The nursing satisfaction in the observation group(72.09%)was significantly higher than that of control group(46.51%).The difference was statistically significant(P<0.05).Conclusions For senile patients of ALVF combined with VAP,the integrated nursing management model can significantly improve the symptoms,enhance the nursing satisfaction and obtain ideal effects.It is worthy of clinical promotion.
