乳核消饮方加减治疗乳腺增生临床疗效及其对血清内分泌激素的影响

摘　　要：目的:探讨乳核消饮方加减治疗乳腺增生的临床效果及其对血清内分泌激素的影响。方法:将初诊乳腺增生女性患者240例按照随机数字表法分为研究组(n=121)和对照组(n=119),对照组给予三苯氧胺联合维生素E治疗,研究组给予乳核消饮方加减治疗,30天为1个疗程,两组均连续治疗2个疗程,经期停止用药。观察两组患者治疗前后临床症状和内分泌激素水平变化情况,两组临床疗效以及不良反应发生情况。结果:治疗后两组患者乳房疼痛、腰酸乏力、月经不调、心烦易怒、畏寒肢冷等临床症状较治疗前改善,差异具有统计学意义(P<0.05),且研究组患者临床症状改善优于对照组(P<0.05)。治疗后两组患者血清泌乳素(prolactin,PRL)和雌二醇(estradiol,E2)水平及E2/P、E2/T比值均降低(P<0.05),血清孕酮(progesterone,P)和睾酮(testosterone,T)水平升高(P<0.05),且研究组降低血清PRL和E2水平及E2/P、E2/T比值、P和T水平升高的作用优于对照组(P<0.05)。研究组总有效率高于对照组,不良反应低于对照组(P<0.05)。结论:乳核消饮方加减可提高乳腺增生患者临床疗效,改善患者临床症状,有助于保持机体内分泌激素水平的稳态,且安全性高。Objective: To discuss clinical effects of modified Ruhe Xiaoyin prescription in the treatment of hyperplasia of mammary glands and its influence on serum endocrine hormones. Methods: All 240 patients were allocated to the study group(n=121) and the control group(n=119) according to random number table method, the control group were treated by tamoxifen and vitamin E, the study group was treated by modified Ruhe Xiaoyin prescription, 30 days were one course, both groups were treated for two courses consecutively, stopping the medication during the menstrual period. To observe clinical symptoms and the changes of endocrine hormones before and after treating in both groups, clinical effects and the incidence of adverse reactions. Results: After treating, clinical symptoms including breast pain, lumbar soreness and lassitude, irregular menstruation, disturbing and irritable, chill,limbs cold and others improved in both groups compared with before treating, and the difference had statistical meaning(P<0.05), and the study group was superior to the control group in the improvements of clinical symptoms(P<0.05). After treating, the levels of PRL and E2, and the ratios of E2/P and E2/T lowered in both groups(P<0.05), the levels of P and T rose(P<0.05), the study group was superior to the control group in the effects of lowering the levels of PRL and E2, the ratios of E2/P and E2/T and lifting the levels of P and T(P<0.05). Total effective rate of the study group was higher than that of the control group, it was lower than the control group in adverse reaction(P<0.05).Conclusion: Modified Ruhe Xiaoyin prescription could enhance clinical effects of the patients, improve clinical symptoms, and it is helpful to maintain the hemostasis of endocrine hormones in human body with high safety.
