<6个月婴儿川崎病联合治疗后因子水平及冠状动脉病变情况的观察

摘　　要：目的探讨不同剂量丙种球蛋白联合阿司匹林肠溶片治疗<6个月婴儿川崎病的临床疗效及安全性。方法将2012年1月至2018年12月在我院儿科治疗的75例川崎病<6个月婴儿随机分为两组,对照组采用丙种球蛋白1g/(kg·d),观察组使用采用丙种球蛋白2 g/(kg·d),比较两组患儿的临床疗效、症状消失时间、各项因子水平变化、冠状动脉病变发生率。结果观察组治疗有效率为97.14%,明显高于对照组的76.67%(P<0.05);观察组退热时间、黏膜充血消退时间、颈部淋巴结肿大消退时间、皮疹消退时间、住院时间明显短于对照组(P<0.05);观察组治疗后CRP、PCT、ESR、WBC、NEUT、PLT水平明显低于对照组(P<0.05);观察组冠状动脉病变发生率明显低于对照组(P<0.05)。结论丙种球蛋白联合阿司匹林肠溶片治疗<6个月川崎病的临床疗效显著,能有效缓解症状,促进炎症的消退,改善血管内皮功能,降低冠状动脉病变发生率,具有积极的临床意义。Objective To investigate the clinical efficacy and safety of combination of gamma globulin and aspirin enteric-coated tablets in the treatment of Kawasaki disease less than 6 months.Methods From January 2012 to December 2018 in our hospital pediatric treatment of 43 cases of children with Kawasaki disease were randomly divided into two groups,control group to aspirin enteric-coated metformin hydrochloride therapy,observation group treated with IVIG combined aspirin enteric-coated metformin hydrochloride,compare the clinical efficacy of two groups of children,the symptoms disappear time,changes in each factor level,the incidence of coronary artery lesions.Results The effective rate of treatment in the observation group was 97.14%,significantly higher than that in the control group(P<0.05).The duration of fever,mucosal hyperemia,cervical lymph node enlargement,rash and hospitalization in the observation group was significantly shorter than that in the control group(P<0.05).The level of CRP,PCT,ESR,WBC,NEUT,PLT in the observation group after treatment was significantly lower than that in the control group(P<0.05).The incidence of coronary artery disease in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The combination of gamma globulin and aspirin enteric-coated tablets for the treatment of Kawasaki disease less than 6 months has a significant clinical effect,which can effectively relieve symptoms,promote the regression of inflammation,improve vascular endothelial function,and reduce the incidence of coronary artery lesions,with positive clinical significance.
