摘　　要：目的探讨不育男性禁欲时间对精子核损伤和精液基本参数的影响。方法选取2018年1月至12月宁波市妇女儿童医院生殖中心诊治的3 702名男性不育症患者为研究对象,按照禁欲时间长短将其分成3组,A组<2天,403例;B组2~7天(WHO推荐组;包含7天),2 953例;C组>7天,346例。采用精子染色质结构分析试验(SCSA)对样本分别计算DNA碎片指数(DNA fragmentation index,DFI)和高DNA可染性(HDS),并进行精液常规参数分析,了解禁欲时间长短与核损伤程度和精液基本参数的相关性。结果三组患者射精量、精子总数、精子浓度、活动精子百分率、不动精子百分率、精子DFI指数比较,其差异均具有统计学意义(均P<0.05)。禁欲时间与射精量、精子总数、浓度、不动精子百分率、精子DFI指数呈正相关(r值分别为0.276,0.234,0.111,0.089,0.187),与活动精子百分率、精子HDS值呈负相关(r值分别为-0.089,-0.067)。结论随着禁欲时间的延长,可能导致精液射精量、浓度、总数、不动精子百分率、DFI指数上升,HDS下降;禁欲时间长短与精液质量有一定相关性,合理的禁欲时间有助于优化精子质量从而提高受孕机率。Objective To study the effect of abstinence time on the sperm nucleus damage and semen basic parameters in infertile men. Methods 3 702 male infertile patients in Women and Children′s Hospital Ningbo form January 2018 to December 2018 were divided into three groups according to the time of abstinence, i.e, group A(<2 days, 403 cases), group B(2-7 days, 2 953 cases) and group C(>7 days, 346 cases). The DNA fragmentation index(DFI) and high DNA stainability(HDS) were detected by sperm chromatin structure analysis test(SCSA). The correlation between abstinence time and the degree of nuclear damage and the basic parameters of semen was analyzed by semen routine parameter analysis. Results There were significant differences in ejaculation volume, total sperm count, sperm concentration, the percentage of motile sperm, percentage of immobile sperm and sperm DFI index among the three groups. Abstinence time was positively correlated with ejaculation, total sperm count, concentration, percentage of immobile sperm and sperm DFI index(r=0.276, 0.234, 0.111, 0.089, 0.187 respectively), and negatively correlated with percentage of active sperm and HDS value of sperm(r=-0.089,-0.067 respectively). Conclusions With the prolongation of abstinence time, semen ejaculation volume, concentration, total sperm count, the percentage of immobile sperm and DFI index may increase, while HDS may decrease. There is a certain correlation between abstinence time and semen quality, and reasonable abstinence time can help optimize sperm quality and improve the probability of conception.
