疏肝益阳胶囊联合达泊西汀治疗肝郁肾虚型原发性早泄的临床观察

摘　　要：目的探讨疏肝益阳胶囊联合达泊西汀治疗肝郁肾虚型原发性早泄的临床效果及安全性。方法选取2017年12月至2018年6月西北妇女儿童医院生殖中心男科诊治的90例原发性早泄患者为研究对象,按照随机数字表将其分为试验组和对照组,每组45例患者。两组患者年龄分别为(30.53±5.06)岁和(31.33±2.77)岁、病程分别为(5.54±3.53)年和(5.04±3.12)年。试验组患者给予口服疏肝益阳胶囊4粒/次,3次/d,盐酸达泊西汀片仅于每次性生活前1~3小时口服30mg;对照组患者仅用盐酸达泊西汀片,用法同试验组,均为4周1个疗程。分别于治疗前后观察两组患者早泄诊断标准评分(PEDT-5)、中医症状评分、临床总体印象改变(CGIC)、阴道内射精潜伏期(IELT)改变并记录有无不良反应,通过统计学分析评估疗效和安全性。结果两组患者PEDT评分治疗前分别为(14.50±1.96)分和(14.10±1.45)分,治疗后分别为(8.20±1.55)分和(9.52±1.81)分,均比治疗前有所改善(均P<0.05),试验组PEDT评分改善比对照组更为明显(P<0.05);两组患者中医症状评分治疗前分别为(13.54±3.01)分和(13.13±2.89)分,治疗后评分分别下降为(9.65±2.86)分和(11.88±2.10)分,中医症状评分均有所改善(P<0.05),试验组改善更为明显(P<0.05);两组患者治疗后CGIC分别为68.2%和55.8%,试验组改善更为明显;两组患者治疗前IELT分别为(0.62±0.28)min和(0.68±0.14)min,治疗后分别为(2.55±1.60)min、(2.24±1.80)min,均比治疗前有所改善,两组间差异无统计学意义;两组患者在IELT方面,其差异无统计学意义;试验组患者不良反应更少。结论疏肝益阳胶囊联合盐酸达泊西汀治疗肝郁肾虚型原发性早泄,临床效果优于单纯应用盐酸达泊西汀片,可以显著改善射精控精能力,同时能够改善患者肝郁肾虚等中医症状、降低达泊西汀片的副作用,值得临床推广。Objective To evaluate the efficacy and safety of Shugan Yiyang Capsule combined with Dapoxetine in the treatment of primary premature ejaculation of Gan(Liver) Depression and Shen(Kidney) Deficiency Syndrome. Methods From December 2017 to June 2018, 90 patients with primary premature ejaculation in the Department of Reproductive Andrology of Northwest Women′s and Children′s Hospital were selected according to the inclusion criteria, and were divided into the experiment group and the control group randomly, each of 45 cases. The ages of the two groups were(30.53 ±5.06) and(31.33±2.77) years old respectively(P>0.05), and the course of disease was(5.54±3.53) and(5.04±3.12) years respectively(P>0.05). The experiment group took orally Shugan Yiyang Capsule, 4 tablets/time, 3 times/d, and 30 mg Dapoxetine 1-3 hours before sex life and the control group received only Dapoxetine, 4 weeks as a course of treatment. PEDT-5, TCM symptom score, CGIC, IELT were observed before and after treatment. The efficacy and safety were evaluated by statistical analysis. Results The PEDT scores of the two groups before treatment were(14.50±.96) points and(14.10±1.45) points, and after treatment were(8.20±1.55) points and(9.52±1.81) points, respectively, which were improved compared to before treatment(all P<0.05), the improvement of PEDT score in the experimental group was more significant than that in the control group(P<0.05);the TCM symptom scores of the two groups before treatment were(13.54±3.01) points and(13.13±2.89) points, and the scores after treatment were respectively The decrease was(9.65±2.86) points and(11.88±2.10) points, and the TCM symptom scores were improved(P<0.05), and the improvement in the test group was more significant(P<0.05). The CGIC of the two groups after treatment was 68.2 % And 55.8%, the improvement in the experimental group was more obvious. the IELT before treatment in the two groups was(0.62±0.28) min and(0.68±0.14) min, respectively, and(2.55±1.60) min and(2.24±1.80) after treatme
