宁夏海原县育龄期妇女健康公平性分析

摘　　要：目的宁夏"创新支付制度,提高卫生效益"医改项目实施前后,以海原县为项目县,分析其育龄期妇女健康公平性及影响因素,以达到改善其健康状况的目的。方法利用农村居民家庭卫生健康询问调查数据,选取2009年和2015年海原县的育龄期妇女作为调查对象,采用集中指数法分析育龄期妇女健康公平性,并利用二分类非条件Logistic回归模型分析健康公平性的相关影响因素。结果新农合方案调整后,海原县育龄期妇女的两周患病率和慢性病患病率均下降(P均<0.01),而自评健康状况很好和好的比例上升(P<0.01),且两次调查的两周患病率和慢性病患病率的集中指数(CI)均为正值。年龄、文化程度及家庭经济收入是影响其健康公平性的主要因素。结论新农合方案调整对宁夏海原县育龄期妇女健康状况有较为明显的改善,但公平性总体表现为亲富不公平性。Objective To analyze the health equity and influencing factors of women of childbearing age by using Haiyuan County as the project county before and after the implementation of the"Innovative Payment System and Improving Health Benefits"in Ningxia,in order to improve the health status of the women.Methods Based on the survey data of household health of rural residents,the women of childbearing age in Haiyuan County in 2009 and 2015 were selected as the survey subjects.The health index of women of childbearing age was analyzed by using concentrated index method,and the relevant influencing factors of health equity were analyzed by using the two classification unconditional logistic regression model.Results After the adjustment of the new rural cooperative medical program,the two-week prevalence rate and the prevalence of chronic diseases of women of childbearing age in Haiyuan County decreased(P all<0.01),while the self-rated health status was good and the proportion was good(P all<0.01),and the concentration index CI was positive.In addition,age,education level and family economic level were the main factors affecting their health equity.Conclusion The adjustment of the new rural cooperative medical program has a significant improvement on the health status of women of childbearing age in Haiyuan County,Ningxia,but the fairness is generally unfair.
