高危妊娠晚期胎儿脐动脉、大脑中动脉血流多普勒参数变化与不良妊娠结局的关系

摘　　要：为探讨高危妊娠晚期胎儿脐动脉、大脑中动脉血流动力学多普勒参数变化及与不良妊娠结局的关系,本研究选取2018年1月~2019年1月于我院行产前检查的高危妊娠孕妇(孕周34~36周)100例,另选取同期相同孕周的正常妊娠孕妇100例作为对照组。采用GE voluson E8彩色多普勒超声诊断仪测量并比较两组胎儿脐动脉搏动指数(PI)、阻力指数(RI)、收缩期末峰值/舒张期末峰值(S/D)值,统计两组孕妇妊娠结局,分析多普勒参数变化与不良妊娠结局的关系。结果显示,高危妊娠组中妊娠结局异常率42.00%,明显高于对照组的5.00%(P<0.05)。高危妊娠组胎儿脐动脉血流参数PI、RI及S/D值均明显高于对照组(P<0.05);而胎儿大脑中动脉血流参数PI、RI及S/D值均明显低于对照组(P<0.05)。高危妊娠结局异常组胎儿脐动脉血流参数PI、RI及S/D值均明显高于妊娠结局正常组(P<0.05);而胎儿大脑中动脉血流参数PI、RI及S/D值均明显低于妊娠结局正常组(P<0.05)。结果表明,高危妊娠晚期胎儿脐动脉、大脑中动脉血流多普勒参数变化对判断胎儿宫内状况、预测不良妊娠结局均具有一定的临床意义。It was investigated in this study that the changes of hemodynamic Doppler parameters of umbilical artery and middle cerebral artery in high-risk pregnancy and their relationship with the adverse pregnancy outcome.100 high-risk pregnant women(34~36 weeks of gestation)who had prenatal examination in our hospital from January 2018 to January 2019 were selected as the observation group,and 100 normal pregnant women(34~36 weeks of gestation)were selected as the control group.The fetal umbilical artery pulsation index(PI),resistance index(RI),systolic peak/diastolic peak(S/D)were measured by GE voluson E8 color Doppler ultrasound,and the results of the two groups were compared,as well as the pregnancy outcomes of the two groups.The relationship between the parameters and the adverse pregnancy outcomes was analyzed.The results showed that,the abnormal rate of pregnancy outcome in high-risk pregnancy group was 42.00%,which was significantly higher than that in the control group(P<0.05).PI,RI and S/D of fetal umbilical artery blood flow parameters in high-risk pregnancy group were significantly higher than those in the control group(P<0.05);PI,RI and S/D of middle cerebral artery blood flow parameters in high risk pregnancy group were significantly lower than those in control group(P<0.05).The blood flow parameters of fetal umbilical artery in abnormal pregnancy outcome group were significantly higher than those in normal pregnancy outcome group(P<0.05);the blood flow parameters of fetal middle cerebral artery in high risk pregnancy group were significantly lower than those in normal pregnancy group(P<0.05).This study demonstrated that the changes of Doppler parameters of fetal umbilical artery and middle cerebral artery in the late stage of high-risk pregnancy have certain clinical application in judging the fetal intrauterine condition and predicting the adverse pregnancy outcome.
