超声评分法与SWE在原发性干燥综合征中的应用价值

摘　　要：目的探讨超声评分法与剪切波弹性成像(SWE)在原发性干燥综合征(pSS)中的应用价值。方法选取有口眼干燥症状、疑诊pSS的患者113例,根据2016年美国风湿病学会(ACR)与欧洲抗风湿病联盟(EULAR)共同制定的ACR/EULAR标准进行诊断,并对患者双侧腮腺、颌下腺行二维超声与SWE检查,分析超声对pSS的诊断价值。结果pSS组74例患者,对照组37例非pSS患者。pSS组超声评分阳性率明显高于对照组(85%vs 59%,P<0.01)。分组后,0、1、2分组中pSS患者腮腺、颌下腺杨氏模量值明显高于对照组(P<0.01)。结论超声评分法与SWE相结合,能够在评估腮腺、颌下腺形态结构改变的同时,进一步评估其弹性改变,对pSS的早期诊断与鉴别诊断具有一定价值。Objective To explore the value of ultrasound scoring system and shear wave elastography(SWE)in the diagnosis of primary Sjogren′s syndrome(pSS).Methods 113patients suspected with pSS underwent the diagnostic work-up according to the criteria published by American College of Rheumatology and European League Against Rheumatism in 2016.All patients accepted ultrasound and SWE on parotid and submandibular glands.Results pSS group included 74patients,non-pSS group included 37patients.The positive rate of ultrasound scoring system in pSS group was higher than control group(85%vs 59%,P<0.01).After grouping,in the groups of 0,1and 2,the Young′s modulus of pSS group were higher than control group(P<0.01).Conclusions Combining ultrasound scoring system with SWE appeared to have certain value for the early and differential diagnosis of pSS.
