肺脏超声中B线在血液透析患者容量状态评估中的应用价值

摘　　要：目的通过分析血液透析前后B线与IVC内径、LVEF、BMI等参数的相关性,探讨肺超中B线在血液透析患者容量状态评估中的应用价值。方法选取我院透析中心36例慢性血液透析患者,使用肺超及超声心动图分别测量透析前后B线、IVC内径及LVEF,比较透析前后三者的变化,并分析透析前后B线与IVC内径、LVEF、BMI等参数的相关性。结果透析后B线评分及IVC内径均低于透析前(P<0.01),透析后LVEF值高于透析前(P<0.01)。透析前B线评分与IVC内径呈正相关(r=0.379,P<0.05),而与LVEF(r=-0.515,P<0.05)和BMI(r=-0.596,P<0.05)均呈负相关。透析后B线评分与IVC内径相关关系消失(P>0.05),而与LVEF(r=-0.597,P<0.01)、BMI(r=-0.599,P<0.01)仍存在良好的负相关性。结论在血液透析患者中,应用肺超B线进行容量评估具有一定的临床价值。Objective To explore the application value of B-line in hemodialysis patients by analyzing the correlation between B-line and IVC diameter、LVEF、BMI and other parameters before and after hemodialysis.Methods 36 cases of chronic hemodialysis patients in dialysis center from were selected.Pulmonary ultrasound and echocardiography were used to measure B-line,IVC diameter and LVEF before and after dialysis,and compare the changes of the three before and after dialysis.The correlation between B-line and IVC diameter,LVEF,BMI and other parameters before and after dialysis was analyzed.Results After dialysis,the B-line score and IVC diameter were lower than those before dialysis(P<0.01),while the LVEF value after dialysis was higher than that before dialysis(P<0.01).Before dialysis,B-line score was positively correlated with IVC diameter(r=0.379,P<0.05),and negatively correlated with LVEF(r=-0.515,P<0.05)and BMI(r=-0.596,P<0.05).After dialysis,the correlation between B-line score and IVC diameter disappeared(P>0.05),while there was still a good negative correlation with LVEF(r=-0.597,P<0.01)and BMI(r=-0.599,P<0.01).Conclusions In hemodialysis patients,it is of clinical value to evaluate the pulmonary capacity by using pulmonary ulstrasound B-line.
