床旁超声诊断新生儿食管闭锁的应用价值

摘　　要：目的探讨新生儿食管闭锁的超声图像特点及诊断价值。方法回顾性分析12例食管闭锁新生儿的临床及超声资料,总结不同类型食管闭锁的超声图像特征。结果12例食管闭锁患儿中,Ⅱ型2例,Ⅲ型10例(其中Ⅲa型2例,Ⅲb型8例),超声诊断为食管闭锁并明确分型11例,诊断准确率91.6%,1例Ⅲb型误诊为Ⅲa型。结论超声能清晰显示闭锁食管近端及远端盲端,测量盲端间距,观察有无合并食管气管瘘,准确判断分型,操作简便、无创,可在床边开展,在新生儿食管闭锁的临床诊断中具有推广意义。Objective To investigate the ulerasonographic characteristics and diagnostic value of ultrasound in neonatal esophageal atresia.Methods Clinical and ultrasonic data of 12neonates with esophageal atresia were analyzed retospectively,and ultrasonic manifestatons of different types of esophageal atresiq were summarized.Results Among 12cases,2were typeⅡesophageal atresia and 10were typeⅢ(including 8cases ofⅢb and 2cases ofⅢa).11cases were accurately diagnosed and typed by ultrasound,whereas 1case of typeⅢb was misdiagnosed as typeⅢa.The diagnostic accuracy of ultrasound was 91.6%.Conclusions High-frequency ultrasound can clearly display the proximal and distal blind end of esophagealand,and measure the distance between the two esophageal blind ends,observe the presence and absence of combined espphageal tracheal fistula accurately determine the dassificat,easy to opende,noninlasive,canke carried owt at the bedside.It has great significance in the clinical diagnosis for neonatal esophageal atresia.
