职业性手毁损伤患者急性应激反应现状及影响因素研究

摘　　要：目的描述职业性手毁损伤患者急性应激反应的现状并探讨社会支持、心理弹性、社会人口学及疾病相关特征对职业性手毁损伤患者急性应激障碍的影响。方法采用横断面调查法,使用一般资料调查问卷、急性应激反应量表(ASDS)、社会支持评定量表(SSRS)、中文版心理弹性量表(CD-RISC)对济南市某三甲医院的250例职业性手毁损伤术后7d内的患者进行问卷调查。结果250例患者有76例存在应激症状,检出率为30.4%;急性应激反应总分(43.99±16.13)分;多元线性回归分析显示:社会支持、心理弹性、受伤时有无恐惧感、性别可解释患者急性应激反应总变异量的63.3%。结论职业性手毁损伤患者急性应激症反应检出率较高,医护人员应重点关注心理弹性水平较低、社会支持较少、女性及受伤时有恐惧感的患者,并对存在上述因素的患者及时进行急性应激反应症状筛查。Objective To describe the condition of acute stress response in occupational hand severe trauma patients,and to explore the influence of social support,mental resilience,social demography and disease-related characteristics on acute stress disorder in occupational hand severe trauma patients.Methods A cross-sectional survey method was used to investigate 250 patients with occupational hand severe trauma within 7 days after surgery in a grade three A hospital of jinan by using general data questionnaire.Acute stress disorders scale(ASDS),social support rating scale(SSRS)and the Chinese version of Conner-Davidson resilience scale(CD-RISC).Results 76 patients had stress symptoms in 250 hand severe trauma,the detection rate of which was 30.4%;The total score of acute stress response was 43.99±16.13;The results of multiple linear regression showed that social support,resilience,absence of fear at the time of injury and gender could explain 63.3%of the total variation of acute stress disorder.Conclusion The detection rate of acute stress disorder in patients with occupational hand severe trauma is high.Medical staff should focus on patients with low mental resilience,less social support,female and fear of injury,and timely screen for symptoms of acute stress response in patients with the above factors.
