摘 要:目的:探讨川崎病(KD)患儿心率变异性(HRV)变化及对冠状动脉损害的影响。方法:选取2015年10月至2018年10月我院收治的80例KD患儿为研究组,根据是否存在冠状动脉损害分为A组(有冠状动脉损害组)和B组(无冠状动脉损害组)各40例,同时选取同期健康体检儿童40例作为对照组。比较各组窦性N-N间期标准差(SDNN)、5 min N-N间期平均值标准差(SDANN)、极低频功率(VLF)、低频功率(LF)、高频功率(HF)、血清氨基末端脑钠肽前体(NT-proBNP)及肌钙蛋白I(cTnI)。结果:SDNN、SDANN、VLF、LF及HF研究组均显著低于对照组,且A组均显著低于B组(P<0.01);NT-proBNP、cTnI水平研究组均显著高于对照组,且A组均显著高于B组(P<0.01);Spearman相关性分析发现,NT-proBNP、cTnI水平与SDANN、VLF及LF呈正相关(P<0.05或P<0.01),与SDNN、HF呈负相关(P<0.05)。结论:HRV指标越低,患儿冠状动脉的损害越大。Objective: To investigate the changes of heart rate variability( HRV) in children with Kawasaki disease( KD) and their influences on coronary artery damage. Methods: 80 KD children admitted to our hospital from October 2015 to October 2018 were enrolled as the study group. According to the presence or absence of coronary artery damage,they were divided into group A( with coronary artery damage) and group B( without coronary artery damage),40 cases in each group. 40 healthy children receiving physical examination during the same period were selected as the control group. The SDNN,5-mininute SDANN,VLF,LF,HF,serum NT-proBNP and cTnI were compared among all groups. Results: The SDNN,SDANN,VLF,LF and HF of the study group were significant lower than those of the control group. The above 5 indexes of group A were significantly lower than those of group B( P < 0. 01). The NT-proBNP and cTnI levels of the study group were significantly higher than those of the control group. The NT-proBNP and cTnI levels of group A were significantly higher than those of group B( P < 0. 01). The Spearman correlation analysis found that NT-proBNP and cTnI levels were positively correlated with SDANN,VLF and LF( P < 0. 05 or P <0. 01),while negatively correlated with SDNN and HF( P < 0. 05). Conclusion: The lower the HRV indexes,the greater the coronary artery damage in children.
