摘 要:目的探讨水通道蛋白4(AQP-4)在肠道病毒71引起的手足口病患儿口腔黏膜中的表达及临床价值。方法选取2016年1月-2019年1月收治的100例肠道病毒71引起的手足口病患儿为病例组,同期入院体检的100例健康儿童为对照组,比较两组受试儿童血清C反应蛋白(CRP)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)水平以及口腔黏膜中AQP-4表达水平。根据中国手足口病防治指南,对病例组患儿病程进行临床分期,比较临床1期、2期、3期患儿口腔黏膜中AQP-4、神经元特异性烯醇化酶(neuron-specific enolase,NSE)表达水平。结果病例组患儿血清CRP、CK、CK-MB水平均高于对照组(P<0.05)。病例组患儿血清、口腔黏膜中的AQP-4表达水平均高于对照组(P<0.05)。治疗前,不同疾病分期患儿口腔黏膜中AQP-4表达水平由低到高依次为临床1期、2期、3期(P<0.05);治疗后,不同疾病分期患儿口腔黏膜中AQP-4表达水平均低于治疗前相应各期,且由低到高仍依次为临床1期、2期、3期(P <0.05)。治疗前,不同疾病分期患儿口腔黏膜中NSE表达水平由低到高依次为临床1期、2期、3期(P<0.05);治疗后,不同疾病分期患儿口腔黏膜中NSE表达水平均低于治疗前相应各期,且由低到高依次为临床1期、2期、3期(P <0.05)。结论口腔黏膜中AQP-4表达与肠道病毒71引起的手足口病发生、发展以及预后相关。Objective To investigate the expression and clinical utility of aquaporin 4 (AQP-4) in oral mucosa of children with hand,foot and mouth disease caused by enterovirus 71.Methods In this study,100 children with hand,foot and mouth disease (HFMD) caused by enterovirus 71 were enrolled from January 2016 to January 2019 as case group and 100 healthy children were used as control group.The levels of serum C-reactive protein (CRP),creatine kinase (CK),creatine kinase isoenzyme (CK-MB),and the expression of AQP-4 in oral mucosa were compared between the two groups.According to the Chinese Hand,Foot and Mouth Disease Control Guidelines,the case group was divided into three stages.The expression levels of AQP-4 and neuron-specific enolase (NSE) in oral mucosa were compared in terms of clinical stage 1,2 and 3.Results The levels of serum CRP,CK and CK-MB were higher in case group than in control group (P<0.05).The level of AQP-4 in serum and oral mucosa was higher in case group than in control group (P<0.05).Before treatment,the expression level of AQP-4 in oral mucosa of children increased with disease stage from clinical stage 1,clinical stage 2 to clinical stage 3 (P<0.05).After treatment,the expression level of AQP-4 in oral mucosa of children with different disease stages was lower than the level before treatment,and from low to high was clinical stage 1,clinical stage 2 and clinical stage 3 (P<0.05).Before treatment,the expression level of NSE in oral mucosa of children increased with disease stage from clinical stage 1,clinical stage 2 to clinical stage 3 (P<0.05).After treatment,the expression level of NSE in oral mucosa of children with different disease stages was lower than the level before treatment,and from low to high was clinical stage 1,clinical stage 2 and clinical stage 3 (P<0.05).Conclusions The expression level of AQP-4 in oral mucosa is related to the occurrence,development and prognosis of hand,foot and mouth disease caused by enterovirus 71.
