双胎动脉反向灌注序列征超声诊断及文献复习

摘　　要：目的:探讨双胎动脉反向灌注序列征超声诊断并复习相关文献。方法:选取笔者所在医院2018年3月26日、2019年3月4日诊断的2例患者,采用GE公司的Voluson E10超声诊断仪诊断双胎动脉反向灌注序列征(TRAP),然后分析超声声像图。结果:病例1超声提示:(1)单绒单羊宫内中期妊娠,双胎,胎儿A存活,胎儿B结构异常。(2)胎儿B无心反向血流灌注。(3)胎儿B单脐动脉。病例2超声提示:(1)宫内中期妊娠,双胎,胎儿A存活,胎儿B结构异常。(2)胎儿B无心反向血流灌注。结论:典型二维、三维、多普勒超声表现能够将有效依据提供给临床早期诊断与宫内干预,从而指导临床选取准确的治疗方式、评估泵血胎预后,值得在临床推广应用。Objective:To investigate the ultrasound diagnosis of reverse perfusion sequence sign of twin fetal artery.Method:Two patients diagnosed by the author’s hospital on March 26,2018 and March 4,2019 were selected for the diagnosis of duplex artery reverse perfusion sequence(TRAP)with GE’s Voluson E10 ultrasonic diagnostic instrument,and then the ultrasound images were analyzed.Result:Case 1 showed that:(1)Single cashmere Intrauterine pregnancy,twins,fetal A survived,fetal B structure was abnormal.(2)Fetal B had no reverse perfusion.(3)Fetal B single umbilical artery.Case 2 showed that:(1)Intrauterine pregnancy,twins,fetal A survived,fetal B structure was abnormal.(2)Fetal B had no reverse perfusion of the heart.Conclusion:Typical two-dimensional,three-dimensional and Doppler ultrasound can provide effective basis for clinical early diagnosis and intrauterine intervention,so as to know the clinical choice of accurate treatment and evaluate the prognosis of the pump fetus,so is worthy of promotion and application in the clinical.
