髂骨内板或外板修复三皮质髂骨取骨区的体会

摘　　要：目的:探讨髂骨内板或外板修复三皮质髂骨取骨区的临床疗效。方法:选取2009年3月-2016年9月三皮质髂骨取骨患者83例,均采用髂骨内板或外板修复三皮质髂骨取骨区,术后定期随访,观察患者是否出现髂骨供区骨缺损、疼痛、盆侧腹壁疝等并发症。结果:全部患者均获得有效随访,随访时间为12~24个月,平均(18.0±5.7)个月;手术时间为25~45 min,平均(29.0±4.0)min;术中出血量为50~250 ml,平均(80.0±29.9)ml;患者术后均未出现髂骨供区骨缺损、疼痛、盆侧腹壁疝等并发症;X线片显示,骨性愈合时间为(2.7±0.8)个月;末次随访时,全部患者髂嵴外观正常且无明显凹陷,取骨区愈合良好,无明显疼痛及压痛,患者活动自如。结论:髂骨内板或外板修复三皮质髂骨取骨区可有效避免髂骨供区骨缺损、疼痛、盆侧腹壁疝等并发症。Objective:To explore the clinical effect of repairing the area of tricortical iliac bone graft with ilium inner or outer table.Method:From March 2009 to September 2016,83 cases of tricortical iliac bone graft were selected,and the area of tricortical iliac bone graft was repaired with ilium inner or outer table.Postoperative regular follow-up was conducted to observe whether the patients had bone defect in the donor area of iliac bone,pain and abdominal wall hernia in pelvic side.Result:All patients were followed up effectively for 12 to 24 months,with an average of(18.0±5.7)months.The operation time was 25 to 45 min,with an average of(29.0±4.0)min.The intraoperative bleeding was 50 to 250 ml,with an average of(80.0±29.9)ml.There were no bone defect in the donor area of iliac bone,pain and abdominal wall hernia in pelvic side and other complications.X-ray showed that the bone healing time was(2.7±0.8)months.At the last followup,all the patients had normal appearance and no obvious depression of the iliac crest,good healing of the osteotomy area,no obvious pain and tenderness,and the patient could move freely.Conclusion:The method of repairing the area of tricortical iliac bone graft with ilium inner or outer table can avoid the complications of bone defect in the donor area of iliac bone,pain and abdominal wall hernia in pelvic side.
