植入式静脉输液港和外周静脉穿刺置入中心静脉导管在白血病患者化疗中的应用比较

摘　　要：目的:探讨植入式静脉输液港(VPA)与经外周静脉穿刺置入中心静脉导管(PICC)在白血病患者化疗中的应用情况。方法:选取上海市杨浦区市东医院2018年6月-2019年4月收治确诊为白血病的68例患者的资料进行回顾性分析,根据患者的置管方法分组,其中VPA组(n=39)、PICC组(n=29)。比较两组一次置管成功率、化疗过程中并发症发生情况和置管后患者的生活质量评分。结果:VPA组手术一次置管成功率为92.31%,大于PICC组的68.97%(X^2=6.236,P<0.05),在生活质量量表8个维度的比较中,只有经济困难这一维度的评分VPA组得分较低,其他7个维度的得分VPA组均高于PICC导管组,但两组间各指标的数据差异均无统计学意义(P>0.05),PICC导管组的并发症发生为41.38%,高于VPA组的10.26%,差异有统计学意义(X^2=8.954,P<0.05)。结论:相比于PICC导管,植入式静脉输液港的成功率高,易于日常维护,且不良反应发生率低,对于改善生活质量具有一定效果。Objective:To investigate the application of venous port acess (VPA) and peripherally inserted central catheter (PICC) in the treatment of leukemia patients.Method:A retrospective analysis was performed on 68 patients with leukemia from June 2018 to April 2019 in Shidong Hospital,Yangpu District,Shanghai.Patients were grouped according to their catheterization methods,including VPA group (n=39) and PICC group (n=29).The two groups of patients were compared for the success rate of one catheter placement,the complications of patients during chemotherapy,and the quality of life scores of patients after catheterization.Result:The success rate of one-time catheter placement in the VPA group was 92.31%,which was greater than 68.97% in the PICC group(X^2=6.236,P<0.05).In the comparison of the eight dimensions of the quality of life scale,only the dimension of financial difficulty was lower in the VPA group.The scores of the other 7 dimensions in the VPA group were higher than those in the PICC catheter group,but there were no statistically significant difference in the data between the two groups (P>0.05).The incidence of complications in patients in the PICC catheter group was 41.38%,which was higher than 10.26% in the VPA group.The difference was statistically significant (X^2=8.954,P<0.05).Conclusion:Compared with PICC catheters,venous port acess have a high success rate,is easy to maintain daily,and have a low incidence of adverse reactions,which has certain effects on improving quality of life.
