摘  要:目的探讨腰椎骨髓化学位移成像(CSI)不同信号指数对骨质疏松症的评估价值。方法选取我院行腰椎MRI检查且排除影响骨质代谢因素的成年患者共102例为研究对象(腰椎从L1~L4),均行常规腰椎MRI及CSI轴位扫描,计算信号指数1~4。根据双能骨密度(DXA)检查(检查间隔不超过2周)诊断结果将研究对象分为骨密度正常组(A)、骨矿含量减少组(B)及骨质疏松组(C),比较各组间信号指数1~4的差异,并分析各信号指数与性别、骨矿含量(BMD)之间的关系。通过绘制ROC曲线分析各信号指数的诊断效能。结果①各信号指数男女性间差异无统计学意义(P>0.05)。②信号指数1(SI)A组(0.62±0.08,P<0.01)与B组(0.51±0.16,P<0.01)均高于C组(0.40±0.08),且A组高于B组(P<0.01)。③信号指数1(SI)与BMD呈正相关(r=0.62,P<0.01)。信号指数1(SI)诊断骨质疏松的ROC曲线下面积为0.81,95%可信区间为0.722~0.899,截断值为0.505,其灵敏度为94.8%、特异度为63.6%。结论腰椎骨髓化学位移成像可以有效诊断骨质疏松症。Objective To evaluate the value of different signal index of lumbar spine bone marrow chemical shift imaging(CSI) in the evaluation of osteoporosis. Methods A total of 102 adult patients with lumbar spine MR examination and exclusion of bone metabolism were enrolled in this study(l L-L4 lumbar vertebrae). All patients underwent routine lumbar MR and CSI axial scans,and the signal index was calculated to be 1-4. The subjects were divided into normal bone density group(A),bone mineral reduction group(B)and osteoporosis group(C)according to the diagnosis results of dual energy bone mineral density(DXA)examination(inspection interval no more than 2 weeks). Differences in signal indices 1-4 between groups,and the relationship between each signal index and gender,bone mineral content(BMD). The diagnostic performance of each signal index was analyzed by plotting the ROC curve.Results ①There was no significant difference between male and female sex in each signal index(P>0.05). ②Signal index 1(SI)normal bone density group(A)(0.62±0.08,P<0.01)and bone mineral content decreased. Group(B)(0.51±0.16,P<0.01)was higher than osteoporosis group(C)(0.40±0.08),bone mineral density group(A)was higher than bone mineral reduction group(B)(P<0.01). ③Signal index A(SI)was positively correlated with BMD(r=0.62,P<0.01). The area under the ROC curve for the diagnosis of osteoporosis by signal index A(SI)was 0.81,the95% confidence interval was 0.722 to 0.899,the cutoff value was 0.505,the sensitivity was 94.8 %,and the specificity was 63.6%. Conclusion Chemical shift imaging can diagnose osteoporosis.
