桂枝芍药知母汤干预对退行性多关节痛早期的疗效及生活质量评价

摘　　要：目的:观察桂枝芍药知母汤干预对退行性多关节痛早期的疗效。方法:采用随机对照的方法,将80例患者分为治疗组与对照组,治疗组采用桂枝芍药知母汤治疗,对照组口服西乐葆,坚持治疗3个月。采用视觉模拟疼痛(Visual Analogue Scale,VAS)量表、生活质量量表(Quality of Life,QOL)分别在治疗前、疗程结束时进行疗效评测。结果:两组在治疗3个月后,VAS评分与治疗前比较,差异有统计学意义(P<0.01),且治疗组疼痛缓解程度优于对照组,差异有统计学意义(P<0.05)。治疗后QOL评分比较,治疗组日常生活能力、社会行动能力、抑郁及焦虑状态与治疗前比较差异具有统计学意义(P<0.05),均得到明显改善;对照组日常生活、社会行动能力与治疗前比较差异具有统计学意义(P<0.05),抑郁及焦虑状态与治疗前比较差异无统计学意义(P>0.05)。治疗结束时,两组临床疗效组间比较,差异有统计学意义(P<0.05)。两组疼痛复发率比较,差异有统计学意义(P<0.05),治疗组复发率低于对照组。两组不良反应发生率比较,治疗组发生率明显低于对照组,差异有显著性意义(P<0.01)。结论:桂枝芍药知母汤干预对退行性多关节痛早期有一定的临床疗效,能显著提高患者的生活质量,其作用机理可能与人体内基因相关信号通路有关。Objective:To observe the effect of the Guizhi Shaoyao Zhimu decoction(桂枝芍药知母汤)on the early stage of degenerative polyarticular pain and the quality of life.Methods:80 patients were divided into the treatment group and control group.The treatment group was treated with the Guizhi Shaoyao Zhimu Decoction.The control group was treated with Celebrex orally for 3 months.VAS and QOL were evaluated before treatment and at the end of treatment.Results:After 3 months of treatment,there was a significant difference of VAS score in groups compared with before treatment(P﹤0.01),and the degree of pain relief in the treatment group was better than that in the control group(P﹤0.05).There were statistically significant differences in ability of daily living,social action,depression and anxiety between before and after treatment in treatinent group(P﹤0.05).There were significant differences in ability of daily living,social action(P﹤0.05),and no statistically significant difference of depression and anxiety between before and after treatment in the control group(P﹥0.05).At the end of treatment,there were statistically significant differences of efficacy,recurrence rate of pain between groups(P﹤0.05).The incidence of adverse reactions in the treatment group was significantly lower than that in the control group(P﹤0.01).Conclusion:The Guizhi Shaoyao Zhimu decoction shows a certain clinical effect on the early stage of degenerative polyarticular pain,can significantly improve the quality of life,and its mechanism may be related to the gene related signal pathway in human body.
