颅内血肿穿刺术与内科保守治疗基底节区高血压性脑出血临界血肿疗效的荟萃分析

摘　　要：目的系统评价颅内血肿穿刺术治疗基底节区高血压性脑出血临界血肿的效果。方法计算机检索国内外数据库中关于颅内血肿穿刺术治疗基底节区高血压性脑出血临界血肿的研究,同时追索纳入文献的参考文献。检索时限均从建库至2018年12月。对纳入研究的质量进行严格评价和资料提取,对符合质量标准的文献进行荟萃分析。统计学分析采用RevMan 5.3软件。结果共纳入6个文献。荟萃分析主要结局结果显示,颅内血肿穿刺术治疗基底节区高血压性脑出血临界血肿在肌力恢复[OR=7.98,95%CI(3.68,17.32)]和整体预后[OR=2.52,95%CI(1.84,3.45)]两个方面,与传统的内科保守治疗方法相比,差异有统计学意义(P<0.05)。结论现有研究显示颅内血肿穿刺术治疗基底节区高血压性脑出血临界血肿疗效确切,偏瘫肢体肌力恢复及整体预后较传统内科保守治疗更好。Objective Systematic evaluation of the effect of intracranial hematoma puncture in the treatment of hypertensive intracerebral hemorrhage critical hematoma in basal ganglia.Methods The computer searched the domestic and foreign databases for the study of the intracranial hematoma puncture to treat the critical hematoma of hypertensive intracerebral hemorrhage in the basal ganglia.At the same time,the references included in the literature were searched.The search time limit is from the construction of the database to December 2018.The quality of the included studies was rigorously evaluated and data extracted,and a meta-analysis was performed on the documents that met the quality standards.Statistical analysis was performed using RevMan 5.3 software.Results A total of 6 literatures were included.The main outcome of the meta-analysis showed that intracranial hematoma puncture for critical hematoma of basal ganglia hypertensive intracerebral hemorrhage in terms of muscle strength recovery[OR=7.98,95%CI(3.68,17.32)]and overall prognosis[OR=2.52,95%CI(1.84,3.45)]were statistically different from traditional medical conservative treatment(P<0.05).Conclusion Existing studies have shown that intracranial hematoma puncture is effective in treating critical hematomas of hypertensive cerebral hemorrhage in the basal ganglia,and hemiplegia limb muscle recovery and overall prognosis are better than traditional medical conservative treatment.
