盐酸纳布啡注射液对剖宫产手术后镇静镇痛效果及负性情绪的影响

摘　　要：[目的]探索盐酸纳布啡注射液对剖宫产手术后镇静镇痛及负性情绪的影响.[方法]本院接受剖宫产手术的128例足月妊娠产妇随机分为两组,64例产妇为观察组,采用盐酸纳布啡注进行静脉自控镇痛;64例产妇为对照组,采用舒芬太尼进行静脉自控镇痛.观察并比较两组产妇的镇静、镇痛效果,测定并比较两组产妇的产后抑郁程度,计算不良反应总发生率.[结果]观察组产妇术后6 h、12 h以及24 h的镇痛评分显著低于对照组产妇,而镇静评分显著高于对照组产妇,两组相比较差异均有显著性(P<0.05).两组产妇产前抑郁评分相比较差异无显著性(P>0.05).观察组产妇产后3 d的抑郁评分显著低于对照组(P<0.05),观察组产妇的产后总抑郁率(7.81%)显著低于对照组产妇(23.44%),两组相比较差异有显著性(P<0.05).观察组产妇的不良反应总发生率为11.76%(8/64),显著低于对照组产妇的59.38(38/64),且两组相比较差异有显著性(P<0.05).[结论]盐酸纳布啡注射液对剖宫产产妇的术后镇痛和镇静具有非常显著的效果,且产后抑郁事件发生的可能性较小,不良反应发生率较低,安全性高,适合临床推广.[Objective]To explore the effects of nabocone hydrochloride on sedation,analgesia and negative e motions after cesarean section in women.[Methods]A total of 128 patients of full-term pregnant women who re-ceived cesarean section in our hospital were selected and randomly divided into two groups.There were 64 cases in the observation group who received intravenous injection of nabocone hydrochloride,while 64 cases in the control group received intravenous sufentanil analgesia.The sedative and analgesic efects of the two groups were observed and compared.The degree of postpartum depression of the two groups were measured and compared as well,while the total incidence of adverse reactions was calculated.[Results]The analgesic scores of 6h,12h and 24h in the ob-servation group were significantly lower than those in the control group;and the sedation score in the observation group was also lower than that in the control group.The difference between the two groups was significant(P<0.05).There was no significant difference in the antenatal depression score between the two groups(P>0.05).The postpartum depression score of the observation group 3 days after delivery in the observation group was signif-icantly lower than that in the control group(P<0.05).The total postpartum depression rate in the observation group was7.81%which was significantly lower than that of the control group(23.44%,P<0.05).The incidence of adverse reactions in the observation group was 11.76%(8/64),which was significantly lower than that in the control group(59.38%,38/64).The difference between the two groups was statistically significant(P<0.05).[Conclusion]Nabocone hydrochloride injection has a good effect on postoperative analgesia and sedation in cesarean parturients,the possibility of postpartum depression and the incidence of adverse reactions are low.It is suitable for clinical popularization.
