环孢素A治疗儿童泛发性脓疱型银屑病的临床效果

摘　　要：目的观察环孢素A治疗儿童泛发性脓疱型银屑病(GPP)的效果。方法对10例接受环孢素A治疗的儿童GPP的临床资料进行回顾性研究,起始剂量为3~5 mg·kg^-1·d^-1,每日2次,至临床症状得到控制,继续治疗1周开始逐渐减量,每2~4周减量0.5~1 mg·kg^-1·d^-1,直至达到最小有效剂量维持治疗6~8周。随访1 a,观察患者临床疗效及不良反应。结果接受环孢素A治疗后,5例患儿临床痊愈,4例患儿显效,临床有效率为90.0%。最常见的不良反应为多毛症,发生率为30.0%,其他副作用如肾毒性、高血压等均未发现。结论在其他药物治疗无效的情况下,环孢素A是治疗儿童GPP效果较好且相对安全的药物。Objective To observe the efficacy on cyclosporine A in the treatment of childhood generalized pustular psoriasis(GPP).Methods A retrospective analysis was studied on ten children with GPP who were treated by cyclosporine A. The initial dosage of cyclosporine A was 3-5 mg·kg^-1·d^-1 and divided into two fractions. This dosage was maintained until the patients had substantially improved. And then it was tapered by 0.5-1 mg·kg^-1·d^-1 every 2-4 weeks until the minimal effective dose was achieved, and then treatment was maintained for 6-8 weeks. Both curative effect and adverse reaction were observed at follow-up period for one year.Results Among the ten patients who were treated by cyclosporine A, five patients were cured clinically and four patients were received marked improvement. The clinical effective rate was 90.0%. The common side effect by the treatment with cyclosporine A was hypertrichosis and the incidence rate was 30.0%, and other side effects such as nephrotoxicity and high blood pressure didn’t appear.Conclusion When other drugs are ineffective, cyclosporine A is effective and safe in childhood GPP patients.
