摘  要:目的探讨代谢综合征(MS)、血脂水平和脂联素(APN)基因多态性与主动脉瓣膜钙化启动之间的关系。方法回顾性以厦门大学附属东南医院2017年2月至2019年4月期间收治的主动脉瓣膜钙化(AVC)患者120例为研究对象,为病例组。另选取同期于该院进行健康体检的120例志愿者为对照组,检测两组体重、身高、血压、血脂[胆固醇(TC)、三酰甘油(TG)]、空腹血糖(FPG)、APN、低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白胆固醇(HDL-C)等相关临床指标。同时培养脂联素rs7649121细胞(AVC组)和空白对照组(不作任何处理)并计数,采用实时荧光定量PCR(RT-PCR)法检测SNPs基因表达情况;采用Logistic回归分析MS、血脂水平和APN基因多态性与主动脉瓣膜钙化之间的相关性。结果与对照组比较,病例组BMI、血压、TC、TG、FPG、LDL-C显著升高,HDL-C及APN水平显著降低,差异均有统计意义(P<0.05)。与对照组相比,病例组细胞侵袭能力显著提高(P<0.05),且细胞中SNPs基因表达下调,mRNA表达水平明显被抑制(P<0.05)。LDL-C、APN rs7649121位点的基因及MS与AVC显著相关(P<0.05)。结论代谢综合征、血脂水平和脂联素基因多态性与AVC启动有关。Objective To explore the correlation of metabolic syndrome(MS),lipid level and adiponectin(APN)gene polymorphism with the initiation of aortic valve calcification(AVC).Methods From February 2017 to April 2019,a total of 120 patients with AVC in our hospital were chosen as the case group,and 120 healthy people in the same time as the control group.Height,body weight,blood pressure,fasting plasma glucose(FPG),APN,low-density lipoprotein cholesterol(LDL-C)and high-density lipoprotein cholesterol(HDL-C)in both groups were measured.The relationship between metabolic syndrome,blood lipid levels and adiponectin gene and AVC was analyzed by Logistic regression.Adiponectin rs7649121 cells(AVC group)and blank control group(without any treatment)were cultured and the number of cells was detected by Transwell method,and the expression of SNPs was detected by real-time quantitative PCR(RT-PCR).Correlation between MS,blood lipid levels and APN gene polymorphism and aortic valve calcification was analyzed by Logistic regression.Results Compared with the control group,BMI,blood pressure,TC,TG,FPG,LDL-C in case group were significantly increased,HDL-C and APN levels were significantly reduced(P<0.05).Compared with the control group,the cell invasion ability was significantly increased(P<0.05),SNPs gene expression was down-regulated and the mRNA expression level of SNPs was significantly inhibited in the AVC group(P<0.05).Logistic regression analysis showed that MS and LDL-C were the risk factors of AVC,and APN was the protect factor of AVC(P<0.05).Conclusion Metabolic syndrome,blood lipid levels and adiponectin gene polymorphisms are closely associated with aortic valve calcification initiation.
