摘 要:目的探讨早发型子痫前期胎盘滋养细胞侵袭相关信号通路表达机制。方法回顾性分析徐州市中心医院2018年2月至2019年1月收治的152例早发型子痫前期患者(74例轻度组子痫前期和78例重度组子痫前期)的临床资料,另外选取同期正常孕妇80例为对照组。免疫组化方法检测子痫前期患者胎盘组织的基质细胞衍生因子(SDF-1)、Wnt1及β-连环蛋白(β-catenin)蛋白进行定位;Western-blot方法检测SDF-1、基质金属蛋白酶(MMPs)、Wnt1及β-catenin蛋白表达情况;侵袭实验检测滋养细胞的侵袭能力。结果SDF-1主要定位在滋养细胞的细胞浆或细胞膜、胎盘绒毛及蜕膜组织中,Wnt1和β-catenin蛋白主要定位在胎盘滋养层细胞;与对照组相比,早发型子痫患者的SDF-1和MMPs蛋白表达水平明显上升,且重度组患者表达量最大,差异具有显著性(P<0.05);与对照组相比,早发型子痫患者的Wnt1和β-catenin表达量明显降低,其次为轻度子痫组,重度子痫组最低,差异具有显著性(P<0.05)。侵袭实验表明,子痫前期患者侵袭能力明显低于对照组,其中重度组患者的侵袭能力小于轻度组患者(P<0.05)。结论随着子痫前期患者病情越重,SDF-1和MMPs蛋白表达越高,Wnt1和β-catenin表达量越低,侵袭能力越低。SDF-1高表达可调控Wnt/β-catenin信号通路,减少Wnt1/β及catenin表达量,降低滋养层细胞的侵袭能力。Objective To explore the expression mechanism of signal pathways related to placental trophoblast invasion in early-onset preeclampsia.Methods The clinical data of 152 patients with early onset preeclampsia(74 patients with mild preeclampsia and 78 patients with severe preeclampsia)were retrospectively analyzed.In addition,80 normal pregnant women with the same period were selected as the control group.Immunohistochemistry was used to localize SDF-1,Wnt1 and beta-catenin proteins in placenta tissues of patients;Compared with normal pregnant women,the expression levels of sdf-1 and MMPs were significantly increased in patients with early onset eclampsia,and the expression levels were the highest in patients with severe eclampsia(P<0.05).The expression levels of Wnt1 and trade-catenin were the highest in the control group,followed by the mild eclampsia group and the severe eclampsia group,with a significant difference(P<0.05).The invasion experiment showed that the patients in the control group were significantly higher than those in the pre-eclampsia group,and the invasion ability of the patients in the severe group was less than that in the mild group,showing a significant difference(P<0.05).Conclusion As the pre-eclampsia patients become more severe,the levels of SDF-1 and MMPs proteins expression were higher,the levels of Wnt1 andβ-catenin were lower,and the invasive ability was lower.High expression of SDF-1 can regulate Wnt/beta-catenin signaling pathway,reduce the expression of Wnt1/beta and catenin,and reduce the invasive ability of trophoblast cells.
