摘  要:目的分析组织因子途径抑制物-2(TFPI-2)水平与鼻息肉的相关性,明确潜在的干预治疗价值。方法选取2017年3月20日至2019年3月20日期间在上海市静安区闸北中心医院住院且经病理确诊为鼻息肉患者50例作为鼻息肉组,另选取同期住院系因鼻中隔偏曲行矫正术并下鼻甲肥大而行部分下鼻甲切除患者50例作为对照组,进行前瞻性队列研究。采用酶联免疫吸附测定(ELISA)方法检测两组患者血浆标本中TFPI-2的蛋白质水平,采用实时荧光定量聚合酶链式反应(qPCR)检测组织标本中TFPI-2的mRNA表达水平以及采用免疫组织化学方法检测组织标本中TFPI-2、肿瘤坏死因子α(TNF-α)、白细胞介素-1(IL-1)和血管内皮生长因子(VEGF)的蛋白表达水平,分析TFPI-2与TNF-α、IL-1和VEGF的相关性。分离鼻息肉组织中的嗜酸性粒细胞,构建上调TFPI-2水平的体外培养鼻息肉组织中的嗜酸性粒细胞模型,观察TFPI-2表达上调后对嗜酸性粒细胞增殖的影响,分析空白对照组、阴性对照组和观察组细胞中TNF-α、IL-1和VEGF表达水平的变化。结果鼻息肉组患者血浆中TFPI-2含量为3.54±1.21 ng/L,与对照组相比显著增高(P<0.05),组织标本中TFPI-2的mRNA相对表达量为2.32±0.93,水平显著提高,差异具有统计学意义(P<0.05)。在蛋白水平方面,鼻息肉组患者的TFPI-2阳性率为56%,高于对照组,且与对照组相比,各类炎症因子的阳性率也表现出显著的不同,差异具有统计学意义(P<0.05)。鼻息肉组患者TFPI-2的蛋白水平和TNF-α、IL-1、VEGF炎症因子存在显著的正相关关系(r=0.642,P<0.05;r=0.535,P<0.05;r=0.769,P<0.05)。TFPI-2表达上调后,嗜酸性粒细胞增殖活性显著下降(P<0.05),细胞增殖指数(PI)和S期细胞比率(SPF)与空白对照组及阴性对照组相比出现下降情况。三组嗜酸性粒细胞中,观察组TFPI-2的蛋白阳性率(100%)最高,并且TNF-α、IL-1和VEGF炎症因子的蛋白质表达出现了抑制Objective To analyze the correlation between tissue factor pathway inhibitor-2(TFPI-2)and nasal polyps,and clarify the potential value of intervention.Methods From March 23,2017 to March 20,2019,fifty patients hospitalized in Jingan District Zhabei Central Hospital and confirmed as nasal polyps by pathology were selected as the nasal polyp group,and fifty patients in the same period because of deviation of nasal septum and hypertrophy of inferior turbinate were selected as the control group.A prospective cohort study was conducted.The protein level of TFPI-2 in plasma samples of two groups of patients was detected by enzyme linked immunosorbent assay(ELISA),and the mRNA expression level of TFPI-2 were detected by quantitative real-time PCR(qPCR).The protein expression levels of TFPI-2,tumor necrosis factor-α(TNF-α),interleukin-1(IL-1)and vascular endothelial growth factor(VEGF)were detected by immunohistochemistry.and the correlation of TFPI-2 with TNF-α,IL-1 and VEGF was analyzed.The eosinophils in nasal polyps were isolated,and the eosinophils model with up-regulated TFPI-2 levels in nasal polyps was constructed.The effect of TFPI-2 expression on eosinophils proliferation was observed.The changes of TNF-α,IL-1 and VEGF expression in blank control group,negative control group and observation group were analyzed.Results Compared with the control group,the TFPI-2 content(3.54±1.21 ng/L)in the nasal polyp group was significantly increased(P<0.05),and the mRNA level of TFPI-2(2.32±0.93)in the tissue specimens was significantly increased(P<0.05).In terms of protein level,the positive rate of TFPI-2(56%)in the nasal polyp group was higher than that in the control group,and the positive rates of various inflammatory factors also showed significant differences compared with the control group(P<0.05).There was a significant positive correlation between TFPI-2 protein levels and TNF-α,IL-1,and VEGF inflammatory factors in patients with nasal polyps(r=0.642,P<0.05;r=0.535,P<0.05;r=0.769,P<0.05).After the expression
