彩色多普勒超声在冠心病心衰介入术后心室重塑状态评估中的价值分析

摘　　要：目的分析彩色多普勒超声在冠心病心衰介入术后心室重塑状态评估中的价值。方法选取2016年1月-2018年5月在河南科技大学第一附属医院治疗的冠心病心衰患者50例为介入术组,以同期健康体检者50例为健康组。所有受试者均进行彩色多普勒超声检查。比较介入术治疗前和健康组的左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)、左室射血分数(LVEF)、二尖瓣环舒张早期与舒张晚期运动峰值速度之比(Ea/Aa)、室壁运动指数(RWMI);比较介入术组治疗前后LVEDV、LVESV、LVEF、Ea/Aa及RWMI值;采用ROC曲线分析多普勒超声检查指标诊断心室重塑的价值,并计算ROC曲线下面积(AUC)。结果介入术组的LVEDV、LVESV、RWMI值高于健康组,LVEF、Ea/Aa值低于健康组(P<0.05);介入术组治疗后的LVEDV、LVESV、RWMI值均低于治疗前,LVEF、Ea/Aa值高于治疗前(P<0.05);LVEDV、LVESV、LVEF、Ea/Aa、RWMI诊断心室重塑状态的AUC值分别为0.990、0.879、0.896、0.793、0.77。结论采用彩色多普勒超声能准确评估冠心病心衰介入术后心室重塑,有助于评价患者行介入术的效果。Objective To analyze the value of color doppler flow imaging in assessing ventricular remodeling after percutaneous coronary intervention(PCI) in patients with coronary heart disease. Methods From January 2016 to May 2018 in the First Affiliated Hospital of Henan University of Science and Technology, 50 patients with coronary heart disease heart failure were selected as the interventional group, and 50 healthy subjects as the healthy group. All subjects were examined by color doppler ultrasound. Left ventricular end-diastolic volume(LVEDV), left ventricular end-systolic volume(LVESV), left ventricular ejection fraction(LVEF), ratio of early and late diastolic peak velocity of Mitral Annulus, and ventricular Wall Motion Index(RVI) were compared between pre-intervention and healthy groups The values of LVEDV, LVESV, LVEF, Ea/Aa and RWMI were compared before and after intervention. The ROC curve was used to analyze the diagnostic value of Dopler’s echocardiographic parameters for ventricular remodeling, and the area under the ROC curve(AUC) was calculated. Results The values of LVEDV, LVESV, RWMI in the intervention group were higher than those in the healthy group, and the values of LVEF, Ea/Aa were lower than those in the healthy group(P < 0.05).The values of LVEDV, LVESV and rwmi in the intervention group were lower than those before treatment, and the values of LVEF and EA/AA were higher than those before treatment(P < 0.05). The AUC values of LVEDV, LVESV, LVEF, Ea/Aa and RWMI were 0.990,0.879,0.896,0.793 and 0.77 respectively. Conclusion Color doppler ultrasound can accurately evaluate ventricular remodeling after percutaneous coronary intervention(PCI) in patients with heart failure. It is helpful to evaluate the effect of PCI.
