曲安奈德治疗葡萄膜炎并发白内障的临床疗效及对复发率的影响分析

摘　　要：目的 探讨曲安奈德治疗葡萄膜炎并发白内障的临床疗效及对复发率的影响。方法 100例葡萄膜炎并发白内障患者作为研究对象,通过随机抽签法分为对照组和研究组,各50例。对照组采用泼尼松龙治疗,研究组采用泼尼松龙联合曲安奈德治疗。比较两组视力恢复情况、术后复发情况与并发症发生情况、治疗前后眼压变化情况及炎症因子水平。结果 研究组患者视力恢复总有效率66.00%明显高于对照组的44.00%,差异有统计学意义(P<0.05)。治疗后1周及1个月,研究组患者眼压分别为(16.80±1.75)、(12.10±1.05)mm Hg(1mm Hg=0.133 kPa),均明显低于对照组的(19.45±2.05)、(15.95±1.60)mm Hg,差异有统计学意义(P<0.05)。治疗后,研究组患者白细胞介素-6(IL-6)、γ-干扰素(IFN-γ)及肿瘤坏死因子-α(TNF-α)水平均明显低于对照组,差异有统计学意义(P<0.05)。研究组患者术后复发率2.00%、术后并发症发生率2.00%均明显低于对照组的14.00%、16.00%,差异有统计学意义(P<0.05)。结论 给予葡萄膜炎并发白内障患者曲安奈德治疗可以取得良好临床疗效,有效改善临床症状,且治疗后复发率及并发症发生率低,具有推广价值。Objective To discuss the clinical efficacy of triamcinolone acetonide in the treatment of uveitis complicated by cataract and its influence on recurrence rate.Methods A total of 100 patients with uveitis complicated by cataract as study subjects were divided into control group and research group by random lottery method,with 50 cases in each group.The control group was treated by prednisolone,and the research group was treated by prednisolone combined with triamcinolone acetonide.The visual acuity recovery,postoperative recurrence and occurrence of complications,intraocular pressure changes before and after treatment and the level of inflammatory factors were compared between the two groups.Results The total effective rate of visual acuity recovery 66.00%in the research group was obviously higher than 44.00%in the control group,and the difference was statistically significant(P<0.05).After 1 week and 1 month of treatment,the intraocular pressure were(16.80±1.75)and(12.10±1.05)mm Hg(1mm Hg=0.133 kPa)respectively in the research group,which were obviously lower than(19.45±2.05)and(15.95±1.60)mm Hg in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of interleukin-6(IL-6),interferon-γ(IFN-γ)and tumor necrosis factor-α(TNF-α)in the research group were obviously lower than those in the control group,and the difference was statistically significant(P<0.05).The recurrence rate after operation 2.00%and incidence of complications after operation 2.00%in the research group were obviously lower than 14.00%and 16.00%in the control group,and their difference was statistically significant(P<0.05).Conclusion Triamcinolone acetonide shows good clinical efficacy for the treatment of patients with uveitis complicated by cataract,and it can effectively improve clinical symptoms with lower recurrence rate and incidence of complications.It has promotional value.
