重症超声CCUE方案在呼吸困难患者急诊会诊中的应用

摘　　要：目的 对重症超声急会诊流程(CCUE)方案在呼吸困难患者急诊会诊中的应用方法及效果进行分析探讨。方法 30例急性呼吸困难患者,在对患者常规诊疗(包括心肺超声检查)的基础上实施重症超声CCUE方案。观察患者病因初步诊疗结果、初步诊断时间、确定诊断时间、初步正确治疗时间及初步诊断准确率。结果 患者经初步诊疗后发现主要病因为急性呼吸窘迫综合征、急性肺水肿、肺炎(肺实变)、心源性休克。初步诊断时间为(16.14±4.83)min,确定诊断时间为(63.28±17.37)min,初步正确治疗时间为(35.27±11.44)min。25例患者初步诊断与确定诊断结果相符,初步诊断准确率为83.3%。结论 通过在呼吸困难患者急诊会诊中应用重症超声CCUE方案,可以准确诊断重症加强护理病房(ICU)重症患者急性呼吸困难的多重致病病因,并且有着较高诊断准确率,能够提高患者治疗成功率,帮助患者及早脱离险情,在患者快速诊断和治疗中能够发挥强大的增益效果,具有临床应用价值与推广价值。Objective To analyze and discuss the method and effect of critical consultation ultrasonic examination(CCUE)regimen in emergency consultation of patients with dyspnea.Methods 30 patients with acute dyspnea were treated with CCUE regimen on the basis of routine diagnosis and treatment(including cardiopulmonary ultrasound).The etiological diagnosis results,preliminary diagnosis time,confirmed diagnosis time,time of preliminary correct treatment and preliminary diagnostic accuracy were observed.Results Preliminary diagnosis showed that acute respiratory distress syndrome,acute pulmonary edema,pneumonia(pulmonary consolidation),and cardiogenic shock were the main causes.The preliminary diagnosis time,confirm diagnosis time,time of preliminary correct treatment were(16.14±4.83),(63.28±17.37)and(35.27±11.44)min respectively.The preliminary diagnosis of 25 patients was consistent with the confirmed diagnosis,and the preliminary diagnostic accuracy was 83.3%.Conclusion In the emergency consultation of patients with dyspnea,CCUE can accurately diagnose the multiple causes of acute dyspnea in intensive care unit(ICU),and has a high diagnostic accuracy,which can improve the treatment success rate of patients,help patients to get out of danger as soon as possible,and play a important role in the rapid diagnosis and treatment of patients.It has clinical application value and promotion value.
