肌骨超声引导针刀进针治疗腰背肌筋膜炎的临床疗效观察

摘　　要：【目的】观察运用肌骨超声精准引导针刀疗法在腰背肌筋膜炎治疗中的应用效果。【方法】将60例腰背肌筋膜炎患者随机分为观察组和对照组,每组各30例,观察组给予肌骨超声引导下针刀治疗,对照组采用传统针刀治疗。比较2组患者治疗前及治疗1、2、4周后简式麦吉尔疼痛量表(The McGill Pain Questionnaire,MPQ)评分,以及改良Barthel指数评定量表(Modified Barthel Index,MBI)评分的变化情况,评价2组患者的并发症及满意度。【结果】(1)观察组总有效率为93.33%(28/30),对照组为73.33%(22/30)。观察组疗效优于对照组,差异有统计学意义(P<0.05)。(2)治疗1、2、4周后,2组患者的MPQ与MBI评分均明显改善(P<0.05),且观察组在改善MPQ评分方面均明显优于对照组,差异均有统计学意义(P<0.05)。(3)观察组患者并发症发生率为6.67%(2/30);对照组为26.67%(8/30)。观察组患者并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。(4)观察组患者总满意度为96.67%(29/30);对照组为70.00%(21/30)。观察组患者总满意度明显高于对照组,差异有统计学意义(P<0.05)。【结论】肌骨超声引导下针刀进针治疗腰背肌筋膜炎能明显减轻患者疼痛,改善患者生活质量,减少并发症发生率,提高患者满意度,疗效显著。Objective To observe the application effect of needle knife therapy for lumbar and dorsal myofascitis under musculoskeletal ultrasound guidance. Methods Sixty patients with lumbar and dorsal myofascitis were randomly divided into observation group and control group,30 cases in each group. The observation group received musculoskeletal ultrasound-guided needle knife treatment,and the control group received traditional needle knife treatment. The changes in scores of McGill pain questionnaire(MPQ)and modified Barthel index(MBI)were compared in the two groups before treatment and after 1,2,and 4 weeks. The complications and satisfaction were evaluated. Results(1)The overall response rate was 93.33% of the observation group, and was 73.33% of the control group. The clinical effect of the observation group was superior to that of the control group,the difference being significant(P<0.05).(2)After 1,2,and 3 weeks,the scores of MPQ and MBI were improved in the two groups(P<0.05),and the improvement in scores of MPQ and MBI of the observation group was superior to that of the control group,the difference being significant(P<0.05).(3)The incidence rate of complications was 6.67%,and was 26.67% of the control group. The incidence rate of complications of the observation group was superior to that of the control group, the difference being significant(P<0.05).(4)The satisfaction was 96.67% of the observation group,and was 70.00% of the control group. The satisfaction of the observation group was superior to that of the control group,the difference being significant(P<0.05). Conclusion Needle knife therapy for lumbar and dorsal myofascitis under musculoskeletal ultrasound guidance has definite effects on relieving pain,improving quality of life,reducing incidence rate of complications,and enhancing satisfaction.
